1112283423 2004-12-21 Orphan No Violations -On site per Columbus permitting request to get latitude and longitude reading for orphan well on property that was formerly owned by Barbara Jean Dowell. Original inspection was carried out by Sam Faust on 1/15/97 and a sketch of the well location was given to me. Dowell has since sold the property to Gene & Pat Schoffner who live in the house on the property. GPS gathered. Several digital photos taken of the well. Well was probably a domestic gas well. Landowner not getting any gas from the well. About a 2 diameter opening in the top of the casing. Gene indicated that a bridge across the Great Miami River is scheduled to be replaced and ODOT is taking some of the field for additional right-of-way. Well is currently about 46 feet from the centerline of River Road according to Fausts drawing. Mr. Schoffner sent right-of-entry aggrement before Christmas 2004. Aggrement received back unsigned on 2/24/05 and sent back for signature on 2/25/05. Signed copy received 3/1/05.
1123597513 2005-08-03 Orphan No Violations -On site for status check on the well being put into the I/O well program. Photos and measurements taken while on site. Title search done earlier in the day at the Shelby Co. Recorders Office. No change to the well. Well case, sticking several inches above the ground is about 8. Well is about 4-5 feet from the new ODOT right-of-way for the replacement of Sulphur Heights Hill bridge over the Great Miami River. Talked to Gene Schoffner while on site. Bridge to be replaced in 4-5 years. Well in horse pasture. River Road old right-of-way is 20-25 feet from well. Power lines present along old right-of-way. I/O well package assembled on 8/4/05 & sent to Columbus. Additional inspection report mailed on 8/8/05 and digital photos e-mailed on 8/8/05 to Jeff Fry.
1190375114 2007-09-20 Plug / Plug Back No Violations -On site for I/O gas well plugging under 2007 Landowner Grant Program. Well had 8 conductor pipe down to the bedrock (depth unknow). No casing was in the well according to Bob Huwer. He indicated that he thought 5 & 5/8 casing had been in the hole but removed in past. A 2 diameter hole wasin the top of the casing.. Met with Bob & Aaron Huwer on site. Hole had been opened & prep to be plugged earlier in the week. Bob said he got some wood out of the well. One piece floated to surface during plugging. Spring Creek Concrete brough 75 sacks of Class A neat cement followed by another truck with 100 sacks. Freshwater displaced in the well during plugging. Good circulation of cement. Area was being cleaned up when I left the site. Bob was to make the cap with ODNR 6-14 ID and planned to excavate around the surface casing, cut it off, cap the well, and fill in the area around the well on Friday, 9/21/07. I will not be able to witness that. Bob to photo the lid, on the well & e-mail me the photo. I will go back up in the next week or so to see that final restoration has been done.

Well was plugged on 9/20/07 by Bob & Aaron Huwer. 175 sacks of class A cement used to fill the 1220 deep gas well. Cement circulated in the approx. 5 & 5/8 hole. Casing was removed in past. On 9/21/09, Huwer cut off the 8 conductor pipe down around 3 below surface, placed plate w/ ODNR 6-14 label on the well, buried the well, and threw some seed down. Digital photos taken by Bob of the cap & conductor/surface casing cut off. Photos mailed & received on 9/27/07. Photos taken of the well location during final restoration inspection on 9/27.
