1228938545 2008-07-21 Production Wells Violations Noted I talked to Harry Boyd on the wells in the bottoms.They are the No.5 and the No.8.I told Harry to get the sites cleaned up and to quit dumping brine in the dikes.He told me that he thought they were Mississippian exempt wells.I told Harry they do not meet the requirements.Harry and Phil Lori went down with a dozer and regraded and moved the tanks out of the dike at bottom of the hill.This site was above the No.8 well.The flow line came over the hill from the well 4-20(api#2768).This well was an Ohio Shale well at the time.
1378863934 2013-09-10 Production Wells Violations Noted Inspection of the well head found the production tubing and rods removed from the well and a reducing swage on the 4.5 production casing in the closed position. Production records show 47 MCF of gas, which does not meet the State minimum of 100 MCF of gas required for a commercial producing well. No proper identification is posted at the well head and tank battery as required by law. Instructions are as follows- Make the said well capable of production or plug the well, post proper identification as required at the well head and tank battery with a permanent sign as required by law, Remove plastic storage tank from next to well head, Clean up oil residue along lease road where the line has been cut and oil was released. ODNR contact, Monroe Co Inspector Bill Stolarik 740- 507 6592.
1379359751 2013-09-16 Production Wells No Violations On this day I received a call from Harry Boyd regarding the violation sent to Him on 09/11/13, in conversation Mr. Boyd indicated that He wanted to change the status to Domestic Ownership, I explained to Him that the well was a commercial well and He was a commercial producer and the well was idle and has been since the last inspector looked at the well and the gas sales chart had not been changed since 11/2012 and the minimum standard of production is 100 MCF of gas per year and the well was not meeting that standard. I also explained to Him that the identification needed to be properly posted on identifiable permanent material. Mr. Boyd was somewhat unclear as to the basic rules of the Division and I suggested He thoroughly read the laws if He wished to be a Commercial Producer. Mr. Boyd requested a on site meeting and I informed Him that the Violation clearly stated His issues.
1439571346 2015-08-14 Administrative Inspection No Violations On this day I was notified by Rob Armstrong who has indicated that He will be the service contractor plugging the well. He indicated he has moved in and pulled the rods and tubing and was going to remove a solid bridge plug set at a depth of 1,380, Mr. Armstrong also indicated that the well was perforated in August of 2008 at a depth of 1,310 to 1,380. I advised Mr. Armstrong we would need to make every attempt possible to get back to the bottom of the well bore and plug the well accordingly implementing proper zonal coverage of all zones perforated. A plugging permit has been issued in response to violation # 1378863934 issued on 09/10/2013.
1440068374 2015-08-19 Plug / Plug Back No Violations Status check inspection on the well plugging process. On the day of my inspection I witnessed a service rig on location owned by a Rob Armstrong rigged up on the hole, the pumping unit has been removed and the flow lines disconnected. Rob Armstrong was on location hooking up a temporary flow line to a 100 barrel tank that has been brought in to use during the plugging process. In discussion with Rob Armstrong he indicated that he would be rigging up to knock out at solid plug set at 1,380 sometime later today. I also question his method of wanting to mix and displace the cement to be used in the plugging process himself. My direct supervisor Brent Bear has been made aware of the up coming proposed cementing method. Inspector Brian Sefcik accompanied me on this day.
1440240953 2015-08-21 Plug / Plug Back No Violations Status check inspection on the well plugging process. On this day I visited the well site and found no work being done. A contractor by the name of Rob Armstrong has his rig on the hole and has made an attempt to knock out the solid completion plug set at a depth of 1,380. During the initial notification to plug call on 08/14/2015 Mr. Armstrong was advised the plug would have to be drove to bottom to access the original perforations indicated on the initial completion reports, Mr. Armstrong indicated the well was perforated in the Injun at a depth of around 1,300, in which I advised those perforations would need coverage also. I also attempted unsuccessfully to contact Mr. Armstrong by phone on this day to get a schedule of when He would be at the well site.
1440600085 2015-08-25 Plug / Plug Back No Violations Compliance notice # 1378863934 follow up inspection. On this day I attempted to visit the well site with the entry gate to the property found locked. A rig owned by a Rob Armstrong has been mobilized and is rigged up on the hole but is currently idle. Several attempts have been made to contact Armstrong by phone unsuccessfully. I did leave a message for the well owner Harry Boyd at 14:07 pm, in which the call was returned by Boyd at 18:42pm. Mr. Boyd did provide the location of a key to allow me to access the property and indicated he would also provide me with a key to allow unrestricted access to the property for inspection purposes. I indicated to Mr. Boyd that I would need timely updates of the status of the work being done and of when they would be working on the well due to my last 2 visits and not being able to locate anyone working and lack of communication from the contractor or well owner. I also informed Mr. Boyd that I would be unavailable until Monday August 31st and no plugging work was to be done until I could be on site to witness the event. We also discussed the contractor ( Rob Armstrong) and his intent to plug the well using a hand mixing trough and trash pump to displace the cement, Mr. Boyd was informed that the cementing methods would have to meet industry standards.
1441475553 2015-09-05 Plug / Plug Back No Violations Compliance notice # 1378863934 follow up inspection. On this day I witnessed a service rig on location owned by Rob Armstrong drill out a solid bridge plug set at 1,380 by the means of a power swivel, a fluid pit was dug and properly lined. Mr. Armstrong will pump the fluid in the pit and properly pack the well off before leaving the location on this day. Upon re-commencing work he will run to the original total depth and then pull the 3.375 tubing and remove the 3.875 Barrel tooth bit and run the tubing back in open ended, at that point plugging will begin.
1441997905 2015-09-11 Plug / Plug Back No Violations 08-17-2015 Rob Armstrong reported he was moving his service rig in to begin the plugging operation and there was a reported cast iron bridge plug set at 1,380 that would need drilled out. 09/05/2015- I was on site to witness the drilling out of the bridge plug. Also noted was perforations between 1,300 and 1,338. 09/11/2015 On site to witness the plug job, Rob Armstrong ran tubing to bottom with a 3.875 tooth bit still on the string and I had to have Him pull the string and remove the bit before plugging was allowed to begin. The tubing was ran back in on this day and Cemenco set 2 plugs one over the Gordon at 2,228 and one over the Big Injun at 1,409. JET Wireline was on location and ran a bond log showing the freepoint of the 4.5 between 425 and 500, the 4.5 was then shot at the closest casing collar at 400. The bottom of the 8.625 surface casing is at 296.
1442591563 2015-09-18 Plug / Plug Back No Violations On this day I witnessed the surface plug set from 406to surface. The rig was unable to pull the 4.5. Cemenco was on location and was able to establish circulation. A 110 sack 14.7 # plug was the set filling the annular and internal of the 4.5 from 406 to surface. Records show the 8.625 surface was set originally @ 296. The previous violation was resolved on this day.
1638693919 2009-04-09 Production Wells No Violations Inspection revealed that well is shut in at this time owner stated that he would has well repaired in the near future and placed back into productionthere is old tubing on site and a area 5x4 (site of old tank) with oil staining present suggested too him to remove stained soil and fertilize affected area and get vegetation established.
1709087159 2010-08-10 Production Wells No Violations On day of inspection Boyd and I and Bret Farnsworth went in to the site. Well is just producing gas. There is 90%vegetation at the site.Boyd stated that pump jack and tubing will be removed and sold.NOV resolved.
2083402455 2015-09-01 Plug / Plug Back No Violations Compliance notice # 1378863934 follow up inspection. On this day a site inspection was conducted to find the service rig owned by Rob Armstrong rigged up on the hole with no one present working on the said well. The well did have tubing hanging in the well on workover slips and the well was not packed off and secured. There was a workover drill stem laying on the tubing rack with a 3.875 bit attached to the stem that has been being used by the service rig operator to attempt to knock out the bridge plug at a reported depth of 1,380. I did receive a phone call from the well owner Harry Boyd (740-213-6910) at 9:22 am who indicated Rob Armstrong has been working periodically on the well and there was possibly a issue with the 4.5 casing collapsed up hole at approximately 800 and Rob Armstrong was possible going to bring a wireline truck to evaluate the well, In informed Mr. Boyd that I was to be informed of any downhole work or evaluation to be done on the well 24 hours prior to the work being done.
