0833244790 2018-01-19 Production Wells No Violations On the day of my inspection I found that the well was equipped with a casing plunger. The flow lines were connected and the valve was open. The production equipment consisted of one 100 barrel steel tank and one horizontal separator. There was no containment dike around the equipment. Correct, legible identification was posted on the tank. The ground was snow covered, but I found no signs of leaks around the wellhead or production equipment.
1134502865 2012-09-28 Production Wells No Violations On the day of my inspection I found that the leak had been stopped by welding a steel containment box to the 8 well head and the tubing. The box contains escaping brine and is equipped with a release valve to dispose of fluid when full. The contaminated soil around the well head was removed and replaces with clean soil. This violation is resolved.
1346338847 2012-08-29 Production Wells Violations Noted On the day of my inspection I found that the well head was open flow from 2-7/8 tubing set on 8-5/8 casing with a casing hanger. The 8-5/8 casing was buried and water was bubbling from the soil around the 2-7/8 tubing. The tank battery consisted of one 100 barrel stock tank and a separator, proper ID was posted. I called Jeff Gress (740-502-2163) to report the leak. He met me at the location to dig around the well head. The water was coming from the annulus between the 2-7/8 tubing and the 8-5/8 casing. He said he will get a service rig out as soon as possible to install a proper well head to control the escape of gas and fluid from the well.
