1252021577 2017-01-04 Production Wells No Violations I was contacted by Frank Riggle of Brilex on 12/29/16 to ask about this well. I had previously reached out to Frank to get a working phone number for Bob Baker. Frank explained that he was working on a deal to take a group of wells from Killbuck Oilfield Service, and this well was included.. I told Frank about the call and what I had found at the inspection. He said that he was going out to look at it. He called me back and said that he had fixed the leak. I contacted the landowner Dean Porr to get confirmation. Mr. Porr confirmed that Frank had been out and fixed the leak. On the day of my inspection I found that the soil had been dug out from around the surface casing and a the valve that connected to the 3/8 inch line for the motor vale had been replaced. The gas leak had been stopped. The identification still needed to be corrected.
1390439411 2017-09-14 Production Wells No Violations On the day of my inspection I found that the tanks and wellhead head been painted and the vegetation had been cleared away, but the identification was still blank. I called Rob Hunter, who claimed that he has his ID signs but they were having trouble with them sticking to the tanks, so he was waiting on an order of signs to hang the stickers on. The well is still showing up as being owned by Killbuck. Rob said that they sent the paperwork and the check into the state weeks ago to transfer the well but he hasnt gotten any confirmation. He said that he would be out here tomorrow to start working on one of the other wells on this lease.
1483023834 2016-12-28 Production Wells Violations Noted I received a call from landowner Dean Porr to report a leaking well on his property. I met Mr. Porr to inspect the well. I found that the well was located approximately 75 feet from the home. The well was equipped with a plunger lift system. The flow lines were connected at the wellhead, but all of the valves were closed. There was an identification sign on the well for permit 3196. I found a continuous flow of natural gas bubbling up through surface water around the well casing. The surface casing was buried and not visible. There was no crude oil around the wellhead and there was no indication of brine stress in the surrounding vegetation. The tank battery is shared with permits 3196, 3294 and 3295. It consisted of one 100 barrel steel tank, one 30 barrel steel tank and one vertical separator. There was an earthen dike around the production equipment. Identification was posted at the tank battery, but the phone number rang busy when I called. The phone number on record in RBDMS was disconnected. I reached out to the local oilfield community and was given a working number for Killbuck Oilfield Services owner Bob Baker. I called twice with no answer. I left messages both times describing the issue and have not received a response. The well owner/operator shall identify and repair the source of the leak within 14 days. Correct identification, complete with a working phone number must also be posted at the wellhead and tank battery.
1496857355 2017-06-07 Production Wells No Violations On the day of my inspection I found that the identification had not been updated. I called Frank Riggle with Brilex because he has previously stated that they were trying to buy the wells. Frank said that the deal was supposed to go through this Friday. I requested that once the wells are in his name he get updated ID on the production tank and at the wellheads as soon as possible so the open violation could be closed
1509120265 2017-10-27 Production Wells No Violations On the day of my inspection I found that correct, legible identification had ben posted at the wellhead. There was still no identification posted at the production tank. I called Rob Hunter, who met me at the tanks and put his new ID sign up. I found a pin hole gas leak in one of the steel lines off of the separator. Rob put a clamp on the line and stopped the leak. The gas leak at the wellhead was repaired on 1/4/17. The compliance notice is resolved

1995915275 2017-08-17 Production Wells No Violations On the day of my inspection I found that identification had not been posted at the wellhead. The ID sign at the tanks was mostly faded and still showed Killbuck information. I had been told by Rob Hunter that Brilex purchased those wells. I called Frank Riggle and left a message in regards to the well transfer and identification.
