0924384688 2015-12-29 UrbanProduction Wells No Violations I was on location for a status check. Due to swelling of Eagle Creek from heavy rainfall, access to the well was cut-off. The tank battery has a 100 barrel production tank, a buried, concrete brine tank and a mechanical separator. There is a dike around the tank battery. An identification sign is properly posted on the tank. The lease road is in good repair.

The equipment on the site consists of the following:



1754851163 2018-07-12 UrbanProduction Wells No Violations I conducted an inspection on this well at the request of landowner, Ms. Mary Lou Lickwar. Ms. Lickwar had concerns regarding her water well integrity and wanted an inspection done on the Lickwar #1 well, to see if it may be affecting her water well. At the time of this inspection, the well was equipped with a plunger lift assembly and the gate valve on the tubing was open. The 2 inch flow line was open. The well was fenced with a locked man door. The tank battery consisted of one 100 BBL tank, a concrete brine tank and a mechanical separator. Valves on the separator were open and the chart dated 7/9/18 showed production. The tank battery was diked. Legible signage was located at the well and tank battery. The lease access was stable. No violations observed. Ms. Lickwars water well was located on the north side of her house. The Lickwar #1 well and tank battery locations were south of the house and downslope from the water well location.
