0514339391 2006-04-10 Production Wells Violations Noted The well was found incapable of production and without an ID. There was a rig on the well. However, there was no evidence of recent activity. There was oil around the wellhead and pooling inside the tank battery dike.
0854270276 2012-05-02 Production Wells Violations Noted Well had no identification. Fluid was escaping from a small leak at a union near the wellhead. An area around the wellhead roughly 15 x 20 had become contaminated. The production line running to the separator was also allowing a small amount of fluid to escape and contaminated a small area around the base of the separator. The dike had water in the lower half of it and the dike wall had been beaten down in places. All leaky equipment shall be repaired or replaced. The dike shall be emptied of fluids. The dike wall shall be rebuilt in order to prevent the escape of oil or brine. All contaminated soils shall be removed and affected areas shall be reseeded and mulched in order to encourage vegetative cover. All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank as to owner, lease name, well number, permit number where available, county, and an emergency telephone number.
1006607622 2013-02-22 Production Wells No Violations Follow up to -854270276. Pumping unit running at time of inspection. Identification at well. Contaminated has been removed from area around well head, area has been seeded and mulched.
1093253760 2013-01-15 Production Wells No Violations Follow up to -854270276. The well is in production, however there is still contaminated soils around the well head. The contaminated soils at the separator have been removed from there original position but pilled on site. I spoke with Mike Merkle of Progressive Oil and we went over cleaning up the site. Pads will be used to remove any contaminates in the puddles at the site and any oily materials. Lime and stray will be laid down. In the spring the soils will be plowed under.

A production line ruptured and flowed roughly 5 Bbl of production fluid into the secondary containment. The dike was able to keep all the fluids contained. Jr. Merkle was contacted and asked to removed the fluids from the dike as soon as possible. A violation was not written because the previous violation of fluids in the dike had not yet been closed.
