1487102730 2017-02-09 Production Wells No Violations I received a call at 10:19 AM from the Austintown Fire Department ( 330-799-8502 ) . They had received a call from a person walking the Mill Creek Hike and Bike about and odor and a sheen of oil inside the tank battery at the above location. Terry Whitehair Northeast Emergency Response Supervisor an myself responded to the call. I called George Stramm from Everflow Eastern and he explained to me that they had discovered a small leak (pen hole) at the 3 fitting at the bottom of the tank. He also had his field foreman Kevin Dow to respond also. The tank at that time had 26 25.8 bbls of fluid. The fluid was removed and the tank was laid over on its side until they can get into the location to remove it. The access road is the property owners yard at this time. There is a dike constructed around the tank battery. The dike had some water and some frozen ice. They will notify me when they remove it. There is a Complaint Status Report # 5529 on this incident
1547491008 2019-01-14 Production Wells No Violations I was on location to perform an inspection on this well and tank battery regarding complaint #5529. A call was received by the Austintown Fire Dept. on 2/8/17 from a hiker who had been walking on the Mill Creek Hike and Bike Trail. The Austintown Fire Dept. Chief said the hiker had seen an oil sheen inside the containment of the dike as the company had been disassembling the tank. The tank was removed by Strawn Oilfield and the site remediated of the oil sheen. A new 50bbl tank has been set in the dike with the existing horizontal separator. The tank battery contains proper identification. The well is a producing plunger lift well with the 8.625 casing plumber to the surface with no violations. The tank battery has been reclaimed and the new production equipment installed. At this time the complaint can be closed.
