Inspection Data

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0419099674 2004-08-18 Production Wells No Violations No violations.
1242707987 2014-04-24 Plug / Plug Back No Violations Upon inspection, it was observed that this site is still largely bare. The pits were filled in. Surface casing was sticking approximately 3 out of the ground and cement could be seen about 6 down. I contacted Tim Wyant of Wyant Exploration Co. to discuss the restoration of this site, and he informed me that AEP waived the final restoration because they have plans to expand their landfill into the area where these wells were plugged. We do not have the landowner waiver on file. I will contact Tim Wyant again to see if he has a copy of this waiver. The restoration will not be passed until the waiver is approved.
1243765831 2013-08-09 Plug / Plug Back No Violations Bowers crew was preparing to set the plug 100-feet in/100-feet out over the bottom of the 7-inch surface casing at 250-feet. Yesterday, 8/8/2013, they bullheaded the Berea plug in the 4-1/2 inch production casing from 1728 to 1716-feet. Prior to this plug, they had removed all of 1-1/2 inch tubing for a total of 1710.5-feet. They set 40 sacks of Class A Cement to cover this interval. Today they tagged the top of this plug at 1300-feet. They used the 1-1/2 inch tubing and circulated the bottom of the surface casing plug with 40 sacks of Class A neet cement. They then came up hole with the 1/1/2 inch tubing and set the top of the 7-inch surface casing circulating 20 sacks of Class A neet cement. This plug was still settling when they mobilized off this site to the next well, the Ohio Power #29, API # 3405320824.
1988745423 2015-06-16 Final Restoration No Violations On my inspection I found that the location had been restored according to 1509.072. I passed the final restoration at this time.
