0613614433 2016-10-31 Production Wells No Violations Today I am onsite responding to a citizen complaint. I notified of a spill on the lease road. When I arrived onsite with inspector Randy Akins, we saw black staining in the middle of the lease road all the way back to the production tanks. This staining varied from approximately 2 feet wide up to approximately 7 feet in places. Some oil globules could be see floating on the standing water in puddles on the road. The tank dike is saturated with both crude and brine. Crude residue reaches approximately 8 inch from the current water surface up the side of the tanks. I notified Alan with AW Tipka Oil and Gas and he met us out at the site. The Terry Whitehair with the Emergency Management Response group was notified of the situation and will have someone on site on 11/1/16.
1478114640 2016-11-01 Production Wells Violations Noted Upon arrival on location I met with Allen Tipka, James Stafford, and Terry Whitehair, Terry and I proceeded by way of the lease road, an observation of the lease road it was determined that brine water and oil was released on the lease road. At that tank battery we found that two of the 210 bbl. tanks had leak and that discharge was contained in the dike area. Clean up was discussed and a plan was agreed on. Pelletize lime and manure will be spread on the lease road. The 210 bbl. tanks will be removed, and the area will then be steamed and the liquids will be hauled away. The dike area will then be bio remedied to complete the cleanup process. The site will need monitoring until completed.

1479487081 2016-11-18 Production Wells No Violations On the day of my inspection I was accompanied by Fred Romeo with ODNR. The site is still in need of some cleanup. I spoke with Kent Adams with Locus to get an update on when the Bio-remediation was going to start. He stated that they have been working on cultures that will stand the colder temperatures. I stated that they have something in place within the next two weeks or we will need to cleanup with a different procedure.
1491387546 2016-10-31 Production Wells No Violations I was notified by inspector Scott Scheiner of a spill on well 34-157-2-4750. I accompanied Scott to the tank battery. Please refer to Inspector Scheiner report.
1500305281 2017-07-17 Production Wells No Violations This is a follow up inspection to a meeting held with Ken Stafford. The cleanup is under way. The tanks and the material in the dike has been removed. Some of the material has been hauled out, but some material remains. The remaining material needs to be hauled out to a landfill. A test for chlorides shows that 4121 ppm chloride remain in the water in the sump hole. The test was performed with high range Quantabs. An attempt to call Ken Stafford has been made to when the remaining material was going to be hauled out.

The tanks have been replaced with one 100 bbl. steel tank and one 50 bbl. plastic tank. The contaminated material have been removed and replaced with new lime stone. I took a water sample using high range Quantabs and resulted in 1121 ppm. chlorides. I will contact Alan Tipka, and discuss further cleanup needed. Additional monitoring is needed.

1590626597 2017-06-30 Production Wells No Violations On the day of my inspection I was accompanied by Terry Whitehair with ODNR, and Ken Stafford with doing work for Allen Tipka, AW Tipka Oil & Gas. It was discussed that more cleanup needs to be done. Quantabs were used for sampling in different areas of the dike. We used low range and found areas of 680 ppm. Chlorides, and use high range and found an area of 1659 ppm. Chlorides. It was explained to Ken Stafford that the water needed to be removed and properly disposed of in a UIC disposal well. Then the straw is to be remove from the dike area and lime is to be applied. At that time, Ken is to inform Inspector Akins and then more sampling is to be done. At that time, it will be determined of more Bio-remediation is needed. Ken was also advised that they have 30 days from todays date to complete cleanup.
1660971320 2016-12-08 Production Wells No Violations This is a follow up inspection to a prior oil and brine spill. The dike are has been started with the Bio-remediation. The area has been covered with straw to insulate the bio cultures. The area will need to have more monitoring.
1696001926 2019-05-31 Production Wells No Violations This inspection was a follow up to a compliance notice number 470686626, issued on 7/8/14, by ODNR Inspector Randy Akins for a tank leak. Upon arrival on location, accompanied by ODNR Inspector Randy Akins, I witnessed the earthen dike area had been remediated. Vegetation is growing on the inside and outside of the dike area. The production equipment included a steel 100 barrel tank, a plastic 50 barrel tank, two horizonal separators and two chart recorders with 2 x 1 inch orifice plates within an earthen dike. The well was equipped with 4.5 inch diameter casing, 1.5 inch diameter tubing, and a plunger lift system. The tubing valves and the casing valves were open. The production tanks were being shared with the Parker 3A ( Permit # 4734 ). The production report was up to date. Proper identification was not present on the new tank. I contacted the well owner to have a new decal put on the tank. He will install the new decals after he paints the new tank.
1959399094 2017-06-28 Production Wells No Violations This is a follow up inspection to a prior oil and brine spill. The dike area has been started with the Bio-remediation. The area has been covered with straw to insulate the bio cultures. The area has had testing and the results are to be forwarded to us by Friday 6/30/2017. The area will need to have more monitoring. A phone call to Allen Tipka for an update.
