0008244975 | 2017-07-17 | Production Wells | No Violations | This well is equipped with 5.5-inch diameter production casing The well appears to be free flowing without any artificial lift technology present at this time. The well is piped to a production area that is fenced in. The area contains the gas processing units and four 400 barrel production fluid holding tanks. There was a flare that was burning on the day of my inspection. Production records in RBDMS are current to 2017 |
0377072636 | 2018-08-01 | Production Wells | No Violations | Status check inspection found a total of 5 producing wells on the pad. All 5 wells along with all production equipment are situated inside a chain link fence enclosure. Proper well identification is posted at the well head and production facility and there is a gate along the lease road to limit access to the well pad. The well is of 5 string construction design with valves -riser pipes and gauges on the 13.375 and 9.625 well head sections. The 13.375 diameter casing pressure is 50 pounds per square inch (psi) and the 9.625 diameter casing pressure is 100 psi, the 5.5 diameter casing pressure is 50 psi and the line pressure is 800 psi. The well is equipped with an artificial lift system. Inside a separate fenced in enclosure is the production equipment consisting of 5 line heaters and gas processing equipment with a continuous flair present on a flair stack while I was on location. The production fluid storage facility consists of 4 steel construction 400 barrel production tanks situated inside a polymer lined steel containment dike area. The well pad has no earthen perimeter berm and the mouse holes do not appear to of been properly plugged after the drilling process with cement. I left my contact information at the well site for the company to contact me concerning the mouse holes. |
0670303036 | 2015-06-03 | Production Wells | No Violations | On the day of my inspection I was accompanied by Anthony Carson. We were following up on the compliance notices that were not yet satisfied. Anthony pointed out that the work that had been completed on the site was not in accordance to the agreed upon plan. There was some residual water outside the sump that contained chlorides that measured 7 on the high Quantab titration strip. The chlorides inside the sump were also elevated and read 8.2 on the high chloride titration strip. Further up the location where the vegetation was impacted previously there were roll offs containing soil that were leaking onto containment not suitable for containing fluids. There were no berms. They had plastic beneath them but nothing to stop the liquid from running off onto the bare ground. The chlorides from this liquid read 8.8 on the high titrations strip. This company has been negligent in properly remediating this site after extensions were granted based on their time requests. I will prepare an Enforcement Action Request. |
0908938596 | 2018-01-11 | Production Wells | No Violations | This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing without any artificial lift technology present at this time. The well is piped to a production area that is surrounded by chain linked fence. The area contains the gas processing units and four 400-barrel production fluid holding tanks. The production records within RBDMS system are current to 2017. |
1042876257 | 2015-05-01 | Production Wells | No Violations | I received an email from supervisor Brent Bear stating that he spoke with Rick Trippel and Bob Worstall and everyone was in agreement to extend the compliance notices on this pad. The new extension date for both compliance notices will be 05/31/2015 at the request of the operator. Operator shall have the sumps installed by 5/8/14. Further sampling will be required including but not limited to water quality and soil impact due to proper measures previously agreed upon be Hess and ODNR representatives not being implemented by Hess. The operator is required to demonstrate through sampling and analytical information that the pad no longer presents an environmental threat. See notices 1422537513 and 169398360 for extensions. |
1190329014 | 2016-05-04 | Production Wells | No Violations | After speaking with Rick Tripple and Jake Glascock it was determined that the Compliance Notice on this well needed to remain open until the pad is properly remediated. Due to Jakes visit to the site 05/03/2016 it is decided that further remediation is required. |
1211485598 | 2015-07-01 | Administrative Inspection | No Violations | I received a call from Anthony Carson stating that the chloride readings inside the cement sump had a Quantab reading of 800 ppm. The second containment area had a reading of 400 ppm and was leaking out of the catch basin. I will contact Steve Smith regarding this issue. |
1331525052 | 2015-04-23 | Production Wells | No Violations
1422537513 | 2015-01-29 | Completion Testing | Violations Noted | I received a call from the operator concerning high levels of chlorides found off the location. I made contact with Rick Trippel to aid in the investigation. During my visit Rick Trippel and Anthony Carson were present on site. We had a meeting with individuals representing Hess Corp. We discussed the areas of concern and the remediation process to follow. While I was present EnviroTrac went over sampling events that had taken place. They stated there was an area of concern with elevated chlorides on the south west portion of the pad. This area is different from the earlier compliance notice on this pad. This area did not have any materials to contain the contaminate and stop it from dispersing onto the ground. However, the company had set up barriers to stop it from dispersing any further down the hill. Rick Trippel and Anthony Carson will be leading the remediation of this site. Operator shall submit a plan of remediation to be approved by the Rick and Anthony. Once approve, the operator shall enact the plan to ensure remediation of the site by the time specified. |
1497894304 | 2014-04-07 | Casing String Witnessed | No Violations | On the day of my inspection the 18.625 casing was ran and cemented. See cement ticket for details. No violations noted. |
1612555864 | 2015-09-21 | Production Wells | No Violations | On the day of my inspection the well appeared to have all the equipment to be considered capable of production. The tank battery contents included four 400 bbl oil storage tanks. The site is still being monitored by the company due to the previous release. |
1693982360 | 2014-07-29 | Drill / Deepen / Reopen | Violations Noted
1749347874 | 2015-06-01 | Production Wells | No Violations | I received an email from Anthony Carson advising that there were still chloride issues on this pad. When I spoke with him via phone on 06/02/2015 he discussed concerns with the proper installation of agreed upon engineering plans not being executed. We plan to visit the site today or tomorrow to follow up on these concerns. Note: Two Compliance Notices still exsist dating 07/29/2014 and 01/29/2015. See reports 1422537513 and 169398360. |
1756604855 | 2016-06-28 | Production Wells | No Violations | This well is currently equipped with the proper equipment to be considered capable of production. During the inspection the well site appeared to be in satisfactory condition. No violations were noted. |
