0874968115 2013-10-23 Plug / Plug Back No Violations I was on location with Inspector Lowdur to witness the pulling of the 2 tubing and the setting of the first cement plug. There was a delay in pulling the tubing as the rods parted inside the tubing and had to be cut off with each joint. A 145 sack cement plug was set and will checked tomorrow.
1030951423 2013-10-24 Plug / Plug Back No Violations I was on location for this well. The well was logged by Appalachian Services and shot off the 6.625 casing 200 down hole where free pipe was shown. 2 3/8 tubing was going to be run into 502 but the hole bridged off around 180. We had to pull out of the hole, and put a cut collar on the tubing to allow us to wash it down. We had to pump on the hole for sometime before we circulated out the bridge. We were able to run down to 502 and set a 150 sack surface cement plug.
1678127553 2015-03-18 Final Restoration No Violations On the date of inspection I am accompanied by SE Regional Manager Brent Bear to inspect the plugged and abandoned well for Final Restoration. The well passes Final Restoration per ORC 1509.072(B).
