0014400489 2014-09-23 Production Wells No Violations I was on location to inspect the clean-up operations from the flow line leak. The upper creek bed has been steamed and looks very good. They are currently steaming out the culvert. I remained on location as they steamed the lower portion of the creek bed and collected the oil. I spoke with Fred Jackson (Aim Energy) and he will be meeting with Bart Ray (OEPA) tomorrow to inspect the location and take any additional measures that are necessary. This clean-up continues.
1086634084 2014-10-02 Production Wells No Violations Follow-up to a flow line leak that occurred on 9/22/14. I inspected the creek bed and Bart Ray (OEPA) had the operator install a large siphon dam at the top of the creek bed about 20 below the leak source. The creek bed is in good condition and the preventative measures are functioning properly. The siphon dams will be left in place and monitored through the spring before they can be removed. No violations.
1432447395 2015-04-02 Production Wells No Violations I conducted a follow-up inspection of this location in regards to a flow line leak that occurred on 9/22/15. I inspected the creek bed north and south of the road. The siphon dam in front of the culvert on the south side of the road has been removed. A larger siphon dam, near the source of the leak, was still in place and function properly. There was a small amount of residual oil skim behind the dam and I will have AIM Energy throw some pads on the skim to remove it. The creek bed is clear and I found no free oil along the course. I will re-inspect the location for any issues after the spring rains have flushed out any residual oil. No violations.
1693718457 2005-08-31 Production Wells No Violations Routine in spection revealed no violations at this time . GPS readings for the wellhead are N4028. 487 and W08114. 858. GPS readings for the tank battery are N4028. 516 and W08114. 562. Note:Common tank battery with permit numbers 1233, 1270, and 1278 . The tank battery consists of two 210 BBL oil storage tanks and one buried steel brine storage tank .
1871735297 2014-09-22 Production Wells No Violations I received a call from Geoff Findlay (Banjo Contracting) about a flow line leak at this well and requested an inspection to assist in cleaning up the release. I arrived on location and found that the flow line leak had migrated through the subsurface and into a dry creek bed and ran downhill 120 to the northwest, through a culvert under Crane Road, and another 60 where it had been blocked by debris in the creek bed. I instructed Fred Jackson (Aim Energy) to report the release to the OEPAs spill reporting line and he did so immediately. I suggested to Geoff that he install a siphon dam above the culvert and dig a sump behind the dam. I told him they should get a steamer truck and flush the product from the creek bed with hot water and collect the impacted water with a vacuum truck. We will do the same on the other side of the road and flush out the culvert and the remaining creek bed. Geoff will begin constructing the upper siphon dam and I will check back tomorrow when steaming operations are conducted.
