There are six wells at this site. The condensate tanks and piping runs were all intact with no leaks. The diesel power unit was found to have excess oil in the spill containment under the engine. The pre-heater unit had no visible issues. While I was finishing my inspection, the contractor was there to clean up the oil under the diesel engine. No oil reached the ground. The lease road is stable condition and the site is well secured with two locked gates. No violations were found. |

My inspection revealed that Patterson Rig# 328 has drilled a 17.5 hole to a total depth of 605. They ran 589 of 13 3/8 casing (54.5 pound per foot) and installed six bowspring centralizers on joints 1,2,3,6,9,12. O-TEX cemented the casing with 540 sacks of Class H cement with 2% CaCl ; .27% Cello-Flake; and 4/10% C-41P. They pumped 115.41 barrels of cement slurry with a density of 15.6 pounds per gallon, 1.2 yield cubic feet per sack, and 5.25 gallons of mix water per sack. They displaced the plug with 84 barrels of water and bumped the plug at 11:14 am with 275 PSI. There was good circulation throughout the job and 24 barrels of cement was circulated to surface. They conducted the casing test at 1100 PSI for 5 minutes and the casing test approved. Inspection of the well site revealed no violations at this time. Note: Drilling rig had trouble with pumps being frozen and steel pits being filled to maximum capacity. The drilling rig had to wait on a roll off tank before O-TEX could conduct the cement job. |

My inspection revealed that Patterson Rig # 328 has drilled a 12 1/4 hole to a total depth of 2240. They ran 2222 of (40# per foot) 9 5/8 casing and installed 15 bowspring centralizers on joints 1,2,3,6,9,12,15,18,21,24,27,30,33,36,39. O-TEX cemented the casing with 790 sacks of Class H cement with 10% NaCl; .27% Cello-Flake; 4/10% C-41P (powder defoamer); 1/10% C35 (dispersant). The cement had a density of 15.6 pounds per gallon, a yield of 1.24 cubic feet per sack, and they used 5.48 gallons per sack of mix water. O-TEX pumped 162 barrels of freshwater to displace the plug. There was good circulation throughout the job and 33 barrels of cement was circulated to surface. They landed the plug at 7:26pm with 960 PSI and went straight into the casing test with 1470 PSI. Casing test was conducted for 5 minutes at 1470 PSI and the test was approved. My inspection of the well site revealed no violations at this time. |

1419681860 | 2015-10-26 | Preliminary Restoration | No Violations | I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water.
