2828177 Primary Facility 2019-01-16 Routine/Complete Inspection No Violations Noted The main access road entrance for this location is off of Cobham Park Rd. in Glade Township, Warren County, PA.

This is a combination oil/gas producing well fitted with an electrically driven pump-jack. The well is plumbed to two buried gathering lines. The main valve from the surface casing is open. The main valve from the production string appears to be closed. There is no evidence of any major leaks or spills around the well head. The stuffing box, however, does appear to be leaking slightly. The well location was constructed as a slight bump-out, on the cut side of its accessing road. The location and access road are currently snow covered. Vegetation, however, can be seen poking through on and around the location. E & S related BMP's seem to have been put in place upon initial construction if the accessing road and location. Regular, continued maintenance, especially with regard to the accessing road, is recommended. The API ID was at one point written on the electrical service box of the pump-jack.
