Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2581346 Primary Facility 2017-04-10 Drilling/Alteration No Violations Noted Cellar Tech auger rig

Drilled a 30” wellbore and set 26” conductor casing at 40 ft. on 4/5/17.

2590284 Primary Facility 2017-05-03 Drilling/Alteration No Violations Noted Fountain Quail Energy Services Rig #6

Moved onto this well on 4/29/17 and resumed drilling at 53 ft. KB. The 18 5/8” casing was set at 651 ft. on 5/1/17 and cemented to surface with 79 bbls returned. The 13 3/8” casing was set at 934 ft. on 5/2/17 and grouted to surface above a basket. Coal was found from 885 ft. to 892 ft. A basket was set at 853 ft. Currently at 1611 ft. and drilling ahead.

2592465 Primary Facility 2017-05-10 Drilling/Alteration No Violations Noted Fountain Quail Energy Services Rig #6

The 9 5/8” casing was set at 2581 ft. on 5/4/17 and cemented to surface with 38 bbls returned. A Casing Annulus Packer was set in the 9 5/8” casing string from 1783 ft. to 1793 ft. KOP was at 4875 ft. TD was at 6136 ft. MD on 5/7/17.

2700395 Primary Facility 2018-02-26 Routine/Complete Inspection No Violations Noted This well has been completed and is now producing.
2705039 Primary Facility 2018-03-07 Routine/Complete Inspection No Violations Noted Today I conducted a routine inspection at the Cogar well pad and the following was observed.

Proper signs were displayed identifying the site. EQT currently has a rig on site and is drilling 1 of 6 new wells. The well heads onsite are contained inside subsurface vaults covered with equipment for the drilling process and could not be visually inspected. The production tanks were in good condition no leaks , rust or defects observed. Secondary containment for the site was intact and maintained.

The site had some stabilization issues on the SW side of the access road. I there was some slides forming inside and outside the LOD. I spoke with Brian Morris from EQT about the issue and we arranged a site inspection on 3/13/18 at 1:30pm to discuss the plan for stabilizing that area.
