Inspection conducted with Rich Gulyas, SWN Construction Foreman. The well permits that have been issued for this pad are the Dropp 1H (115-21560), 2H (115-21561), 3H (115-21562), 4H (115-21563), 5H (115-21564), and the 6H (115-21565). All perimeter BMP’s in place to include red lined additions. Pad is 95% complete. No violations noted.
2349719 Primary Facility 2015-03-04 Routine/Complete Inspection No Violations Noted Arrived at Southwestern's Dropp 4H at 230pm on March 4th, 2015. Location was completely snow covered besides stacked containment mats and a cattleguard in place around the presumed cellar. Nothing could be seen in cellar because of snow besides a yellow tag slightly protruding from the snow. Site was properly ID'd, will return after snow thaw.

2375701 Primary Facility 2015-05-27 Incident- Response to Accident or Event No Violations Noted Arrived at the site at 11:40 AM in response to a reported spill an estimated of three (3) barrels, one-hundred and twenty-six (126) gallons of fresh water mixed w/ hydraulic fracturing additives; mainly friction reducer. During the inspection, I was accompanied by Seth Tratthen of SWN. I also met with Reggie Thomas, SWN completions company man. Mr. Thomas informed me that frac operations were completed for the Dropp 4H well yesterday; 5/26/15. The general area where the release occurred is located at N- 41.88104; W- 075.60711. Some frac sand was observed on containment where the release occurred. Conductivity readings in pad surface water near where the release occurred, which may be the lowest point of the pad, were in the range of (800-1,000) microSiemens/cm. The water on the pad surface in this area did not contain a visible sheen or discoloration. There is no evidence the fluid released out of the tank escaped secondary containment and no evidence of compromised containment integrity indicating an impact to the pad surface.

The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water above the surface casing with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 539psi on the production casing and 424psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:05p.m.
