The site was last inspected by company personnel on 11/19/2015 with the following comments “No new issues visible. Currently raining.” On 11/5/2015 the following comments were observed in the onsite inspection log, “1. Small crack in foundation above fill slope, center of pad. Monitoring. 2. STs holding water. 3. Minor erosion on fill slope appears unchanged. Monitoring.” |

As of today’s inspection, the site appears to have been restored recently. ST1 and ST2 have been replaced by a large infiltration basin below the fill slope of the pad. Due to recent above average precipitation events multiple areas of erosion are visible on the sidewalls of the basin and off the corners of the pad. Please see attached photos. Shell is in the process of fracking 4 Utica wells on the pad, the 21H, 22H, 24H and 26H. The rack is scheduled to be complete in 4 to 5 days. Water was observed in the ditches surrounding the pad. Puddles on the pad surface were tested for conductivity and found to be in normal range. The Infiltration basins below the fill slope of the pad were holding water. CTS#333786 can be closed out at this time. No violations are noted at this time. I departed the site at 1415 hours. -ELN |
