SWEPI notified the Department on 11/16/15 that an estimated 5 barrels of stimulation fluid was released during frac operations. The force of the release tore the liner containment, allowing approximately half of the fluid to migrate onto the ground. Fluid was observed on the ground along the south side of the containment. Additional puddles were observed outside of containment on the north and east sides of the pad. Violations cited against the 23H well (117-21755). TLT

SWEPI monitored rock-lined channels and other stormwater BMPs from 11/17/15 - 11/23/15. Monitoring confirmed impact limited to pad surface. Affected pad material excavated by 11/25/15. Approximately 15 gallons of triethylene glycol was release to the pad surface from a tote with a faulty ball valve. Free liquid immediately soaked up with absorbent pads and effected soil scraped up for disposal. No evidence of staining on pad surface. One violation cited against the 23H well (117-21755) however, violation immediately corrected based upon prompt cleanup of spill. TLT

2688404 Primary Facility 2018-01-30 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection inspected the SWEPI LP Miller 394 well pad located in Delmar Township, Tioga County on January 30, 2018 at 1345 hrs. The weather was 20 degrees and sunny with 3-4 inches fresh snow on the ground. There was no activity onsite or company personnel during my inspection.

The 1Vwell was drilled in 2010 and placed on in active status. The 1H, 3H, 5H, 7H were spud, but not drilled to total depth. The 23H and 25H wells are currently in production.

PCSM BMPs were observed onsite. No violations or issues were noted. I completed the inspection at 1445 hours.

2829332 Primary Facility 2019-01-18 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Miller 394 Well Pad in Delmar Twp, Tioga County on 1/18/2019 at 1110 hours. The weather was 30 degrees, partly cloudy and 1/4 inch of snow covered the pad.

There are currently 2 wells onsite that appear capable of production, the 23H and 25H. They were both tagged with the appropriate API numbers. The well site remains wells vegetated and stabilized. No erosional issues were observed. It appears that a frac tank was just placed on pad this morning. It sits within proper containment.

No violations noted today, I departed the site at 1135.
