2295882 Primary Facility 2014-08-13 Routine/Complete Inspection No Violations Noted The purpose of the inspection was to follow-up on a report from the operator that a coal seam was encountered. The access road has been constructed and previously disturbed soils have been seeded and stabilized. Some fair growth was noted at the time of the inspection. The pad site has been cleared and three sedimentation basins have been installed and are functioning properly. The sedimentation basins were essentially filled and were showing signs of erosion on the upslope side. The basins are functioning properly and no other erosion was noted on or downslope of the site. Please monitor the site and address potential E&S concerns as they arise, clearly the site was not designed to remain in this condition over an extended period of time. The construction remains suspended until an overburden analysis can be performed and additional options for construction and mitigation of potential acid-producing rock can be evaluated. In the meantime, the coal seam is not exposed and the site has been temporarily stabilized.

2708069 Primary Facility 2018-03-15 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection of the SGL 075 Pad A was conducted March 15, 2018. At the time of inspection there were 5 wells on the pad; four are plugged conductors and one is an active conductor awaiting plug and abandonment. The operator has submitted a Notice of Intent to Plug this remaining conductor, the #2731 (081-21507). The four plugged wells all had 2" candy cane vent pipes installed on a conductor cap as the well monument. The remaining active conductor was capped. No obvious issues were observed during this inspection.
2719886 Primary Facility 2018-04-13 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection on the SGL A pad was conducted April 13, 2018 following the plugging of the last remaining active well on the pad; the #2731 well (conductor only). At the time of inspection, the 2731 appeared to be plugged and covered with gravel. There were 4 previously plugged wells on the pad (conductors), all of which had vent monuments installed. No obvious issues were observed with these wells.
