Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0553593274 2013-06-19 Preliminary Restoration No Violations The producing unit consists of the following elements: A production wellhead and storage tank battery. The production wellhead is fitted with a mechanical plunger lift system and is located 200 feet east of the tank battery. The tank battery system includes 2 100-barrel storage tanks, a 50-barrel poly tank, mechanical separator and associated appurtenances. The tank battery is enclosed in a chain link enclosure. The enclosure is locked and the secondary containment is good. Tank and well spacing is in accordance with 1501:09. HB 278 equipment is present. No problems or violations noted. OK for preliminary restoration.

1141464783 2013-07-15 UrbanProduction Wells No Violations The producing unit consists of a wellhead and mechanical plunger lift unit and an aboveground storage tank (AST) battery with one steel 140-barrel production tank, one steel 20-barrel brine tank, mechanical separator, and metering run. Both the wellhead and battery are within the same chain-link enclosure which is locked. The unit ID is present and meets requirements. 1501:9-9-05E safety equipment is present. The battery secondary containment is good. No evidence of leaks or spills.
1956303617 2017-11-29 Production Wells No Violations I inspected this well as part of routine inspection activity. The well and battery are located on a larger, farm parcel. At the time of this inspection, I found the following: a wellhead plunger-lift unit and associated battery in separate chain-link enclosures. The fittings and connections were secure and free from leakage or problems. Fittings and connections at the battery were secure. The load-lines were plugged and locked. ID signage was present and current. The secondary containment was intact with no signs of leakage, spills, or problems. The shared-use access road was in good condition. The well was in active production. The chart recorder was active. Production reports and insurance were current.
2053176808 2012-03-19 Production Wells No Violations My inspection found this well in good repair. This site passed Preliminary Restoration on 08/08/2011. The tank battery is in need of HB-278 equipment and Gary McIntyre was informed of this issue.
