At the time of the previous inspection, I had noted that the load-line valves were not secure with locks as required and the wellhead enclosure was missing. At the time of this inspection, I observed the following: the access road to the well and battery, which is comprised of a gravel-paved road that enters from Ravenna Road and is shared with the residential parcel and travels 50 feet to the well and battery, was in good condition with no drainage problems or wash-outs. The battery enclosure gates were locked and secure. The secondary containment was intact. Fittings and connections at the wellhead and battery were secure and free from leakage. I walked the areas surrounding both the well and the battery and found no signs problems or impact to the surrounding in-situ features. I did observe the following maintenance and safety deficiencies; the load-line valves remained unsecure and require locks, the ID signage had not been updated to reflect the recent change in ownership, and the wellhead enclosure remains dismantled. The owner has indicated that the deficiencies will be fixed but will not provide a timetable. |

The wellhead is located behind the referenced house on Ravenna Road. The wellhead and battery are located in a gravel drive and parking area behind the house. The well is comprised of a mechanical plunger-lift system which had been enclosed but the sides of the board-on-board enclosure have fallen down. Fittings and connections at the wellhead are secure. The battery is comprised of one 210-barrel steel production tank and one 100-barrel brine tank and separator unit in a separate board-on-board enclosure. Fittings are secure. Load-outs are locked. The containment dike is intact and dry. I walked the areas outside of the wellhead and battery including areas down gradient topographically and did not observe any signs of impact or drainage problems associated with the unit. The access road is unpaved by well drained and passable. |

In addition, the steel brine tank was displaying advanced and substantial corrosion on the sidewall requiring intervention before failure of the tank integrity. The ID signage has not been updated to reflect the change of ownership that occurred over 12 months prior to this inspection. I have contacted the company on several occasions since the issuance of the compliance notice. The company has reported that problems with personnel and/or lack of funds, have prevented them from being in compliance. |

Fittings and connections at the wellhead were secure. The 8.625-inch casing is open and plumbed into the 4.5-inch production casing. The former board-on-board enclosure has been removed. The battery is comprised of one 210-barrel steel production tank, one 100-barrel steel brine tank and separator in a separate board-on-board enclosure. Hatches were closed and sealed. Secondary containment was present and intact. Fittings and connections were secure and free from signs of leakage or problems. I walked the areas surround both the well and battery and did not observe any signs of impact relative to the production activity. Signage was present. I noted the following safety issues: The load-line valves were not locked and the wellhead enclosure must be replaced. The owner must remedy the following by 04/03/2016: secure the load-line valves with locks. Repair and secure the wellhead enclosure.
