0496355463 2016-02-17 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

8 400 bbl. Production tanks 4 Producing wells 4 Sand separators

2 Horizontal heater-treaters. 3 Compressors 2 NGL tanks There was approximately 4 to 5 inches of snow on location at the time of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.

0755377701 2014-09-12 Casing String Witnessed No Violations This well was spug on 9/11/14. Rocky Mountain Drilling drilled 120 of 26 hole and set 120 of 20 casing. Universal cemented with 250 sacks of class A cement. See the casing ticket for the details.
1417617060 2014-12-03 Drill / Deepen / Reopen No Violations On the day of my inspection I found the crew continuing to rig up and move in equipment. They project that they will be spudding by this evening.
1805793827 2014-12-05 Casing String Witnessed No Violations On the day of my inspection I found Nomac 72 preparing to run 13 3/8 inch casing. Schlumberger will be doing the cementing. The casing is set at a depth of 555 feet. Established circulation and circulated 36 bbls of good cement returns. Plug bumped and held pressure as required.
