Inspection Data

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
000PL16145 Preliminary Restoration No Violations Preliminary Restoration done information from Data Point
0123077071 2011-02-22 Production Wells No Violations FOLLOW UP TO NOV # -1447518160 A follow up inspection with Brent Bear revealed lease road needs graded. Earthen dike needs to be worked on. Proper identification needs to be placed on the well. OAC 1501:9-9-05 Production needs to be submitted. ORC 1509.062A Verbal notification was given by Brent Bear and he gave them 30 days.
0163433101 2017-12-19 Production Wells No Violations My inspection revealed the well status has not changed. The well remains idle and no attempt has been made to bring the well into compliance. The well consists of a pumping unit, 2-100 barrel tanks and a poly tank. The polish rod was dull and the pulleys are rusty.
0339991476 2014-07-17 Production Wells No Violations I met with landowner Glenneth Griffith. He stated he wants the well put back in production or plugged. Logan Gas purchased the well from Oil Enterprises in 2010. No attempt has been made to bring the well into compliance. I have set a meeting up with Logan Gas to gain compliance.
0822838252 2016-01-14 Production Wells No Violations Compliance notice follow-up. The well consists of a pumping unit, 2-100 barrel tanks and a poly tank. The well remains idle and incapable of production. New pictures were taken.
0979961678 2011-01-18 Annular Disposal No Violations The Division received a landowner complaint alleging non-production from this well. An inspection revealed idle well equipped with a pumping unit and engine. The casing valve was closed and valve for pumping tee left open. The flowline from this well is routed to an approx 140 bbl tank with no knockout. A 650 gallon polyeurothane tank was found not connected and would serve as saltwater tank. An old 100 bbl tank corroded and containing holes also, observed but not connected. An area below the dike location, and along the woodline, dead trees, were observed, which may have been oilfield related at one time. However, the impact did not appear to new. The landowner alleges that previous owner would have been responsible for brine release. RBDMS shows well transfer on 2/18/2010. Records also indicate that 2005 was the last year for commercial production. The displayed identification does not show the current well owner information and contains the wrong permit number for the well. Erosion was observed along the lease road and will require grading. Due to the location, the landowner recommends that lease road be addressed in the Summer. After consulting with Mr. Robert Stonerock, South Region Oil & Gas Supervisor, Mr. Jim Kilbarger will be given a verbal notice to comply within 30 days. If follow-up inspection reveals deficiencies still exist, a Compliance Notice will be issued. Mr. Jim Kilbarger was informed of the inspection results and 30 day requirement to comply, on
1301684204 2011-02-22 Annular Disposal No Violations A follow-up inspection to verbal notification was conducted with Mr. Andy Thomas. During the inspection, it was noted that the well status remains unchanged since the last inspection. Mr. Thomas will issue a Compliance Notice to the well owner. Note: Winter weather conditions at time of inspection.
1447518160 2011-02-22 Production Wells Violations Noted A follow up inspection with Brent Bear revealed lease road needs graded. Earthen dike needs to be worked on. Proper identification needs to be placed on the well. OAC 1501:9-9-05 Production needs to be submitted. ORC 1509.062A Verbal notification was given by Brent Bear and he gave them 30 days.
1548421882 2019-01-24 Production Wells No Violations I conducted a compliance notice follow-up on this well today. Inspector Andy Thomas issued a compliance notice to Logan Gas Inc. on 3/7/2011 for the well to be plugged. The last year for reported commercial production was 2005. My inspection revealed that the well was equipped with a pumping unit and motor. The polish rod was dull and did not indicate recent signs of production. The lease road was overgrown with vegetation and had substantial ruts. I took photos of the well and tank battery. On 1/25/2019 I met with Jim Kilbarger at his office and discussed his plans for this well. Mr. Kilbarger told me he intends on producing the well. Mr. Kilbarger said he will have someone go out and look at the well location and let me know when he will be able to put it back in production. Time of inspection: 2:10 PM. Weather: 28 degrees F, Overcast
