1109878376 2018-08-07 Field Inspected, Well Not Found No Violations The purpose of my inspection was to conduct a status check inspection on well #2-0439. Inspector John Fleming was present during the inspection. Vegetation overgrowth prevented access and locating the well head. Inspector Fleming and I met with landowner Edward King. Mr. King stated that the he had purchased the well from the former operator and provided me with documentation signed and notarized by the former well owner. I communicated to Mr. King that I would forward his contact information to ODNR-DOGRM surety and bonding section to facilitate proper transfer and documentation of the well. I will follow-up with surety and bonding and schedule an exempt domestic transfer inspection when requested.
1580510970 2018-12-18 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-0439. Inspector John Fleming was present during the inspection. Access remained stable and drivable through the R.W. Sidley slag dump. Correct identification was present at the well head. The well was located within a locked and privacy screened chain-link fence enclosure. The well head was below grade. The 4.5 production casing was equipped with a swage and a 2.5 ball valve that was in the open position. The 2.5 line was reduced down to 1 and plumbed through two (2) pressure regulators and a vertical drip. The regulated pressure indicated 25 psi. No leaks were observed. Inspector John Fleming was working with the well owner (Mark Mcginty) and the land owner (Edward King) to complete a Form 7 transfer. No violations were observed during this inspection.
1904519312 2004-08-31 Preliminary Restoration No Violations Met on site with Jeff Bryon of Belvior Oil & Gas. Replaced the bad regulator. Gas leak stopped.
