0337027039 | 2017-02-08 | Production Wells | No Violations | Inspected production well on 02/08/2017. The tank battery consists of 1-210 bbl steel tank, 1-50 bbl steel tank, and 1-horizontal separator, located within an earthen dike. Well identification is clearly displayed at the tank battery. The 50 bbl steel tank is rusted through in spots, and must be emptied and removed. Jim Shoots with Killbuck Creek Oil was notified of this issue and will have it addressed as promptly as weather permits. The well is equipped with a flanged wellhead with T, swedge, and flowline. No other problems were noted at the time of the inspection. |
0984095543 | 2015-01-02 | Production Wells | No Violations | Pumping unit well with no apparent issues at this time. |
1201900492 | 2017-02-08 | Preliminary Restoration | No Violations | Inspected production well on 02/08/2017. All drilling equipment and debris have been removed, pits closed, and the well placed into production. Preliminary restoration passes. |
1664177569 | 2018-05-02 | Production Wells | No Violations | Inspected production well on 05/02/2018. The tank battery consists of 1-210 bbl steel tank, 1-50 bbl steel tank, 1-horizontal separator, 1-drip, and 1-meter box, located within an earthen dike. Well identification is clearly displayed at the tank battery. The 50 bbl steel tank was noted to be rusted through in areas on the back side, but did not appear to be in-use at the time of the inspection. The well is equipped with a flanged wellhead with T, swedge, and flowline. No other problems were noted at the time of the inspection. |

1995698653 | 2016-07-19 | Production Wells | No Violations | Inspected production well on 07/19/2016 to follow-up on Chiefs Order #2016-175. The tank battery
consists of 1-210 bbl steel tank, 1-50 bbl steel tank, 1-horizontal separator, and 1-meter box, located within an earthen dike. The 50 bbl tank is ruster through, and needs to be removed. Well identification is posted at the tank battery. The chart in the meter box appears to show production, however the chart is not dated, and thus the date of production is unclear. The well is equipped with a flanged wellhead with a T, swedge and flowline. The well is not shut-in at the wellhead, indicating that it may still be producing, and may therefore be in violation of the Chiefs Order. Findings will be forwarded to ODNR DOGRMs legal staff for further action. No other problems were noted at the time of the inspection. |
