2092312357 2018-05-23 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-2538. ODNR-DOGRM Northeast District Supervisor Stephen Ochs was present during the inspection. Access to the well was on foot due to wet conditions. Correct identification was present at the well head. The 8.625 well head was below grade, with the annulus plumbed into the flow line. The 4.5 production casing was equipped with a plunger lift assembly. Production valves were in the open position. No leaks were observed. The horizontal separator was located along the lease road and remained in good repair and contained correct identification. A current 7 day production chart was in the meter box. A stable earthen dike that was contained meteoric water was present around the production tanks. Tanks within the dike consisted of one (1) 100 bbl steel production tank and one (1) 50 bbl steel brine tank. The production tank remained in good repair and contained correct identification. Valves were closed, locked and plugged. The brine tank remained in good repair. The inlet valve was closed. No violations were observed during this inspection
