They cleared the 7 inch diameter casing down to the open hole section of the wellbore. The drill pipe was tripped out and the 4.5 inch diameter casing was ran back in with a Parmaco packer. The packer was set at 2015 feet. The bottom of the 7 inch diameter casing is set at 2071 feet. This puts the packer within the 100 foot allowed from the injection zone.

They have milled a obstruction from 1500 feet to 1590 feet. The wellbore was then circulated with salt water to remove any obstructions they remained in the wellbore. Line tested at 890 psi, at 5 minutes 885 psi, at 15 minutes 870 psi. Inspected the containment dike, tanks, unloading pads, injection pump. Tested the specific gravity of the brine from inside the pump house. The specific gravity was 1.13.

A tracer wire was installed on the entire length of the line. 3 line has flanged transition fitting on each end with stainless steel line connecting to the facility and the injection tubing. Tested the 3 line at 650 psi for 15 minutes with no loss in pressure.
1437160581 2015-07-16 Surface Facility Construction No Violations Request for injectivity test was authorized on July 6, 2015. 48 hour duration test. Authorization expires on July 30, 2015. The injection pump was turned on and injection test started at 12:12pm on 7-16-15. The test was shut off at 1;12pm due to electrical and remote monitoring issues. The high pressure limit switch was not operational. The injectivity test has been rescheduled. There were no leaks observed at the wellhead and the injection pressure did not exceed the max injection pressure on the permit.

Drill rig is currently on the well at 2430 feet to mill out a piece of an old packer. Milling operations are shut down while drill mud is brought up to weight for well control. BOP is installed on the well casing. All fluid is being stored in tanks on site. Brine trucks are hauling circulated fluid from the open tank to the K&H facility for disposal.
1500989638 2017-07-25 Salt Water Injection Well No Violations Key Drilling on site, they have released the packer and began pulling tubing at 8:00 AM. The packer became stuck at approximately 1500 feet. Key drilling pulled 50,000 pounds for .5 hours in an attempt to free the packer with no success.
1501169460 2017-07-27 Salt Water Injection Well No Violations KLX Energy Services provided the tools to remove the packer that was stuck at 1530 feet. The hydraulic jars removed the packer. When the packer was brought to surface, it was found the slips, rubber and end collar remained in the well bore. I instructed well owner Central Environmental Services to have Key Energy service rig attempt to recover the remaining pieces of the packer.
1501256010 2017-07-28 Salt Water Injection Well No Violations The parmaco packer was set today at 1519 feet. The annulus was tested at 521 PSI for 15 minutes. The pressure drop was 7 psi in 15 minutes.
1505752779 2017-09-18 Salt Water Injection Well No Violations K&H Partners has excavated at the wellhead to expose the injection tubing. The tubing was cut off and a new 4.5 inch diameter fitting was crimped onto the cut off tubing. New 4.5 inch diameter stainless steel casing was plumbed into the wellhead. The new casing was pressure tested at 600 psi for 15 minutes with 5 psi loss. The test passed as the allowable loss 30 psi.
1506604150 2017-09-28 Salt Water Injection Well No Violations Key Energy arrived on site on 9.27.17 and attempted to pull out the packer. I did not witness this attempt. I arrived at 8:15 Am and met with Central Environmental Services engineer Robert Rudoff. Robert informed me the packer is stuck at approximately 1524 feet. They have attempted to lower the packer to see if this will allow the packer to release. KLX Energy Services has used bumper jars to push the packer down hole. After several attempts the packer was moved down hole where it was released and removed. KLX ran a gauge ring down hole and found a obstruction at 1500 feet. The obstruction will be pushed down hole below where the packer will be set. This work will be performed on 9.29.2017. If the obstruction cannot be pushed down hole, then they will mill out the obstruction.

The injection tubing had 355 psi and the annulus had 4 psi. The containment dike was free of fluids. This satisfies the compliance notice issued on 3.1.2018.

A diversion ditch was constructed upslope from the well that is channeling the surface runoff from the wood line away from the well site. The access road is graded and water bars have been installed. Good vegetative cover has been established on the well site and all affected areas along the sides of the access road.

2135893234 2015-04-08 Drill / Deepen / Reopen No Violations Witnessed 117 of new 13 3/8 conductor casing set in 137 of 17 1/2 bore hole with 105 sacks of Class A cement. A centralizer was ran on the first collar down. Circulation was established and cement was seen at the surface and did not fall back. The 9 5/8 casing that was to be used for the surface job was inspected and rejected due to collar irregularities and questionable condition. New casing will be purchased and inspected before it is ran into the hole.
