1528994163 2018-06-11 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-3149. On 6/08/2018 a local resident reported a gas release from the tank battery. ODNR-DOGRM Emergency operations and response coordinator Fred Romeo responded to the incident with local fire personnel. RBDMS indicated reported production was current. Access to the well and tank battery remained drivable and stable. The well was located within a locked chain-link fence enclosure. The 8.625 well head was below grade. The 4.5 production casing was equipped with a plunger lift assembly. The master gate valve was closed. No leaks were observed. A gauge fitted to the tubing string indicated 180 psi. A domestic supply connection was present. A stable earthen dike that was free of fluid was present around the production equipment. A locked chain-link fence enclosure was present on top of the dike. Equipment on location consisted of one (1) mechanical separator, one (1) 140 bbl production tank and one (1) 50 bbl poly brine tank. The separator was shut in. At the time of my inspection, I did not witness any impact from the gas release. The production tank remained in good repair and contained correct identification. Valves were closed, locked and plugged. The hatch lid of the tank remained unbolted and removed from the tank body. The poly brine tank remained in good repair. The inlet valve was closed. I will contact Bocor Holdings to determine the status of repairs and source of failure.
