0514656632 2014-11-13 Orphan No Violations This well is under contract to be plugged for the Orphan Well Program by Davis Well Service. At the time of the inspection the well had been excavated to a depth of 10 below grade on 11-12-14. There was 6 casing inside 8 casing with a collar. A 10 long by 8 diameter casing was threaded into the existing 8 collar. The remaining hole was backfilled with soil provided by the landowner. Drilling started with the 5 bit and cable tool at 1 pm and an obstruction was encountered at a depth of 15. Drilling was stopped for the day at a depth of 25. Samples of the contaminated soil from the well were collected. These samples were taken to ERS in Bowling Green, Ohio on 11-14-14. A crew from OBriens Safety Services was on location to monitor gas levels. There were readings as high as 9 ppm Hydrogen Sulfide and 70 LEL in the well. Levels around the well and work area stayed between zero and 2 ppm of Hydrogen Sulfide throughout the cleanout.

At the time of the inspection drilling/cleanout continued at a depth of 45 around 8:30 am. Drilling stopped at about 62 at 4:00 pm. 0619196104 2014-12-02 Orphan No Violations This well is under contract to be plugged for the Orphan Well Program by Davis Well Service. On 12-1-14 a pocket of gas was encountered that caused one of the members of the plugging crew to become immediately sick. The gas quickly dissipated and the well showed no sign of gas after that. The crew member was treated on location then transported to the local hospital where he remained until 12-2-14. The gas was encountered at 155 and a second plug was encountered at 416. At the time of the inspection drilling/cleanout continued with a power swivel and 2 7/8 tubing with a 4.5 roller cone bit at a depth of 416 around 8:30 am. Drilling stopped at about 550 at 4:00 pm. Drilling was slow to allow for good circulation of the existing bentonite plug. Gas levels at the well and in the working area remained at zero throughout the day as noted by the personal gas meters worn by the plugging crew.

1223322017 2008-09-29 Production Wells No Violations Contacted by Rick Judge of Siteworks Drilling on behalf of Neal Johannsen the landowner. The landowner and neighbor reported odors over time and a burn area in field where crops will not grow. Landowner excavated burn area under supervision of DMRM and located the following items during the excavation process. Ground that was stained, segments of 2 inch pipe, segments of 1 inch pipe with regulator, severed orange gas line in direction of former landowner residence of 2717 Court Rd. A 9 foot segment of 8 inch casing cut off with a cutting torch, a 10.5 foot segment of 8 inch casing with swage and 2 inch cap that was intact, and remnants of numerous Baroid hole plug bags. During excavation Bentonite was observed in the former excavation. The original well was located 10 feet below grade. The borehole was filled with bentonite to within 2.5 feet of the top of 8 inch steel casing. A 12 inch drain pipe was fitted over the well and backfilled for later location. Some gas was leaking from well. During site activities it was reported by the current landowner that there would be no negotiations for purchase with a well on the property. Also, reported the previous landowner may have contacted Tibboles Well Drilling, Inc. a water well driller in Bellevue, Ohio for plugging of the well.

Hartland Twp, SE1/4 SE1/4 Section 3, Lot 11 Use gas meter to collect readings; On ground near pipe head, 4% LEL and 1 ppm H2S. At top of pipe, 49% LEL and 19 ppm H2S. In pipe, 82 % LEL and 29 ppm H2S. Wind about 5 to 7 mph from west, temp upper 50s. Rain over night filled lower portion of excavation. Well is degassing uncontrolled.

1375807059 2013-08-05 Orphan No Violations An inspection of this orphan well revealed the release of H2S from inside a 14.5 outside diameter black corrugated pipe. The fluid within the pipe was near the surface and contained a strong and constant flow of gas to the surface. Note: A layer of gray film was also observed on the surface of fluid. The well was monitored with a gas detector equipped with an automatic pump, a probe and filter. The following concentrations were noted during the inspection: H2S= 30; CH4= 3.54; and the LEL was exceeded. The well was situated on property designated as agricultural field with no crops planted at this time. The location contained two residential dwellings within approximately 500. The well location did not contain any overhead obstructions or oilfield debris. A GPS reading was obtained and recorded for the well and potential entry off Court Rd. Central Office will be notified of H2S status for this township.

At the time of the inspection drilling/cleanout continued with a power swivel and 2 7/8 tubing with a 4.5 roller cone bit at a depth of 550 around 8:30 am. A total depth of 809 was reached and the well was circulated for one hour to allow for good circulation of the existing bentonite plug. Gas levels at the well and in the working area remained at zero throughout the day as noted by the personal gas meters worn by the plugging crew. The well was shut in and cement was scheduled for 8:30 am the next morning.

1442640870 2014-12-04 Plug / Plug Back No Violations This well is under contract to be plugged for the Orphan Well Program by Davis Well Service. At the time of the inspection PetroSet Cementing was on location and 160 sacks of 50/50 Pozmix was pumped down the well in two stages. The cement was pumped through the combination of 2 7/8 and 2 3/8 tubing used during cleanout. Good circulation was maintained throughout pumping but cement was stopped short of the surface to allow for the removal of the riser pipe. The cement level inside the well was unknown but residual cement on the pipe suggests about 50 from the surface. Gas levels at the well and in the working area remained at zero throughout the day as noted by the personal gas meters worn by the plugging crew. Please see Plug Report for details.
1545044847 2014-12-05 Plug / Plug Back No Violations This well is under contract to be plugged for the Orphan Well Program by Davis Well Service. The well was plugged on 12-4-14 with 160 sacks of 50/50 Pozmix. At the time of the inspection the cement level inside the well was at around 80. The remaining space was filled with 20 sacks of Quickcrete mixed on location by the contractor and poured in from the surface. The identification plate was placed in the well and the 10 by 8 riser pipe was removed. This puts the top of the plugged well at ten feet below grade. Gas levels at the well and in the working area remained at zero throughout the day as noted by the personal gas meters worn by the plugging crew. Please see Plug Report for details.
