Area needs repair seeding completed before preliminary restoration can be passed.
0829307504 2013-04-03 Production Wells No Violations My inspection found the well to be equipped with a pump jack. The annulus valve was closed. The well and tank battery are in good repair. The tank battery consists of four 210 BBL production tanks, four 100 BBL brine tanks, and four mechanical separators. The proper identification is located on the tank battery. The lease road is in stable condition. The lease road and tank battery are shared with permit numbers 2-2092, 2-2047, and 2-2048.
0855829888 2010-06-04 Drill / Deepen / Reopen No Violations On site for BOP test. Pits were in good repair. Lease road is soft but stable. Drainage controls along lease road are in place. Operator has placed silt fence and straw bails per my request at drainage discharge points along the lease road. BOP test: Start time 1427, 800 psi on BOP 200 psi on the hole. Test end time 1445, zero loss of pressure. Per Jim Dixon source of fresh water was hauled in.

1275925211 2010-06-04 Drill / Deepen / Reopen No Violations On site for surface cement. Hole #2: TD at 508 11 inch. Surface pipe: 8.625 new R-3 20lb pipe set open ended, 463 of pipe set at 475, 12 landing joint. No issues running pipe. Operator broke circulation ran 200lb of gel and one bag of hauls ahead of cement. 175 sacks of Class A 3% CaCl cement ran, 6 BBLs of good cement returned to surface. Cementing of CND was waived bedrock was at 20 feet. Operator did set 38 of New R-3 32lb pipe. Pipe was cemented during surface. Pits were in good repair. Lease road was stable. Operator has installed drainage controls along lease road. Fire extinguishers on the rig were in the green, but two years past inspection. Advised Jim Dixon to have extinguishers inspected. Hand rails around rig were up. Safety signs posted on the dog house.
1275937683 2010-06-02 Drill / Deepen / Reopen No Violations Site is under construction. Pits were clay with a shale bottom, had operator pack clay on the bottom of the pit to prevent cutting of the liner. Requested drainage controls be installed along the lease road and location to prevent off site erosion and sedimentation. First 300 of the lease is rocked and stable, remainder of the lease road is raw clay. Permit was in the dog house.
