The well was equipped with a plunger lift system with the valves in the open position. Gauges present read 440 pounds per square inch on the casing and 430 pounds per square inch on the tubing with annular pressure monitoring in place. 1316878148 2018-04-25 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a plunger lift system with production valves in the open position. Annular pressure monitoring was present. The production equipment consisted of four 400 barrel tanks, four line heaters, and four horizontal separators. A gas compression skid, horizontal vent scrubber, and a flare stack to capture tank vapors were located on the west side of the pad. A flame was present coming from the flare stack that was roughly 4 feet high at global positioning coordinates 40.05750 -81.26691. Identification was present and production records were current.
1363555185 2013-03-16 Drill / Deepen / Reopen No Violations On site to witness running and cementing of 20 surface casing,H&P Drilling rig 387 ran 420 of casing and used 1-shoe and 1-basket and 5 centralizes and once casing was on bottom they ran 5 drill pipe inside of casing and attempted too sting into shoe so they could cement casing in place ,unable to sting into shoe and drill pipe was partially hung in hole once free they pulled pipe and then attempted to pull 20 casing and they were unable to do so, then decision was made to run impression tools inside of casing to verify that casing is possibly collapsed.

Verified that casing is collapsed on first joint in hole and it is collapsed 8.5 in, company is trying to make decision on how to remedy problem.

1769281732 2015-12-10 Production Wells No Violations On the day of my inspection I witnessed the production equipment included a line heater, a bulk separator, a flash gas compressor, a fuel gas skid, a vapor recovery system, 3-400bbl condensate tanks and a 400bbl produced water tank within containment and a flare stack. The well was equipped with Crown wellheads and a combination of Neeco and Cameron valves making up the production tree. Gauges present read 200psi on the casing with proper identification and current production records.

The production equipment consisted of four horizontal indirect line heaters, four horizontal separators, one gas compression skid, four 400-barrel tanks, and a flare stack. Identification was present and production records were current.
