The production equipment included a sand trap; a GP unit and 2-400 barrel purge tanks. Fluids are separated at the GP unit and then transferred to the Hayes CGF. Gauges present read 350 pounds per square inch on the tubing and 425 pounds per square inch on the casing. Annular pressure monitoring was present along with proper identification and production records being current.
0131778323 2015-03-02 Preliminary Restoration No Violations On the day of my inspection I witnessed that the well was equipped with Sweco wellheads and production trees. The production equipment included a sand trap, a GP unit and 2 400bbl purge tanks. Fluids are separated at the GP unit and then transferred to the Hayes CGF. Tubing and Casing pressure was at 2600psi with the intermediate and surface casing at 0psi. The site passed preliminary restoration according to ORC 1509.072A.
0210490246 2018-06-11 Production Wells No Violations The purpose of my inspection was a status check. The well was equipped with a production tree style wellhead with a plunger lift system. Production valves were in the open position. A gauge was present on the production tubing reading 520 pounds per square inch, 790 pounds per square inch on the production casing, 1000 pounds per square inch on the 9.625 inch diameter intermediate casing, and 0 pounds per square inch on the 13.375 inch diameter surface casing. The production equipment consisted of two 400 barrel tanks, six sand traps, six gas production units, and one indirect line heater. An above ground storage tank was situated in the Northwest corner of the pad. The identification was located at the wellhead.
0411374401 2016-07-05 Production Wells No Violations On the day of my inspection accompanied by ODNR Inspector Scott Scheiner, I witnessed that the well was equipped with Sweco wellheads; a production tree and a plunger lift system with valves in the open position. The production equipment included a sand trap; a GP unit and 2-400bbl purge tanks. Fluids are separated at the GP unit and then transferred to the Hayes CGF. Gauges present read 610psi on the tubing and 845psi on the casing. Annular pressure monitoring was present along with proper identification and production records being current. We completed a walk around with Eclipse Well Tender Bradley Paisley.

Gauges present read 520psi on the tubing and 650psi on the casing.

The production equipment included a sand trap; a GP unit and 2-400 barrel purge tanks. Fluids are separated at the GP unit and then transferred to the Hayes CGF. Gauges present read 265 pounds per square inch on the tubing and 375 pounds per square inch on the casing. Annular pressure monitoring was present along with proper identification and production records being current. An Above Ground Storage Tank (AST) was present on the northwest corner of the well pad supplying fresh water to the Frank Miller pad for frac with support being provided by Great Lakes. Water was field tested revealing chlorides of <33 parts per million, pH 7 and 60 degrees.

The production equipment included a sand trap; a GP unit and 2-400 barrel purge tanks Fluids and gas are separated at the GP unit and then transferred to the Hayes CGF. Gauges present read 420 pounds per square inch on the tubing and 440 pounds per square inch on the casing with annular pressure monitoring in place. Proper identification and production records were current. The Above Ground Storage Tank (AST) remains onsite along with 2-500 barrel empty frac tanks within containment.

The production equipment included a sand trap; a GP unit and 2-400 barrel purge tanks. Fluids and gas are separated at the GP unit and then transferred to the Hayes CGF. Gauges present read 355 pounds per square inch on the tubing and 505 pounds per square inch on the casing with annular pressure monitoring in place. Proper identification and production records were current. The Above Ground Storage Tank (AST) remains onsite along with 2-500 barrel empty frac tanks within containment.
1333989705 2014-03-04 Drill / Deepen / Reopen No Violations On the day of my inspection, I witnessed Schlumberger break circulation with fresh water, then mix and displace 462.0 sacks of Class A cement with a weight of 15.6 and a yield of 1.20, 135 barrels of fresh water was used for displacement, I witnessed 22 barrels of cement back to surface.
1394024782 2014-03-04 Casing String Witnessed No Violations On the day of my inspection, I witnessed Alpha Hunter Rig #9 and crew attempt to run the last joint of 8.625 surface casing in the well bore, the casing was hung at 455 due to the red rock, Schlumberger circulated the hole with fresh water while trying to wash the pipe in, the minimum requirement to be set by the state was 300, A decision was made to cement the surface casing at 455.
1842207190 2016-05-24 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with Sweco wellheads, production trees and a plunger lift system. The production equipment included a sand trap; a GP unit and 2-400bbl purge tanks. Fluids are separated at the GP unit and then transferred to the Hayes CGF. Gauges present read 1080psi on the tubing and 1275psi on the casing. Annular pressure monitoring was present along with proper identification and production records being current. Discussion was held on the readings with Eclipse Lease Operators Bradley Paisley and Danny Hreha.
