0202189318 2015-11-30 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

0319106249 2013-04-02 Drill / Deepen / Reopen No Violations I was on site to check the status of the construction of the well site and lease road. My inspection revealed that the construction crew is finishing the well site and lease road. Nomac 74 is suppose to be moving onto this well site after the conductors are set. No violations at this time.
0627689796 2013-04-22 Casing String Witnessed No Violations I was onsite to witness the cementing of the 13 3/8 surface casing set to 807. Nomac 74 had some trouble establishing circulation earlier in the job with fresh water, they received 2 additional loads of water to ensure the well could be circulated and that there was enough fresh water on site for the cement job. The rig was able to establish circulation and extra fluid loss material was used in the 25 barrel gel spacer. The cement was scaled at 15.7ppg before it was pumped down hole. Circulation was lost from 0-45 barrels, reestablished from 45-85 and then lost again until roughly 38 barrels into displacement. In total 20 barrels of cement weighing 15.3ppg were returned to surface and it was a successful cement job.
1092466297 2013-04-22 Drill / Deepen / Reopen No Violations I was onsite to check the status of the drilling on this location. The rig was at 807 and was preparing to rig up casing crew. They estimate that the surface cement job will start at around 9pm tonight.
1828941482 2016-03-01 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road was equipped with a gate and was locked. The #1H well is in production. The well is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.

The equipment on the pad consists of the following: 6 - 400 bbl production tanks, three labelled as containing produced water, and three labelled as containing condensate. 1 Low pressure vertical separator located inside the dike 6 Producing wells 6 Sand separators 6 Gas processing units 1 Heat Treater 1 Vapor Recovery Unit

2010678438 2018-12-18 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate and intermediate/production annular gauges displayed readings of zero psi. I performed a walk-around inspection of the production pad and erosional controls appeared in-place. No violations were observed at the well or production pad at the time of inspection.
