0098438422 2017-10-30 Production Wells No Violations I was on location to conduct a status check of this well. The well is equipped with a plunger lift assembly. The pad is equipped with eight producing wells. Jersey barriers were installed around the wellhead. The tank battery consisted of 16, 400 BBL production tanks, and was surrounded by a steel dike. Other production equipment on the well pad consisted of:
8 sand separators
8 gas processing units
1 low pressure vertical separator
1 compressor unit	
1 heat treater

Identification was posted at the access entrance, the well, and the tank battery. The lease road was in good repair. No violations were observed at the time of inspection.

0467781456 2015-10-23 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of twelve, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

2045686186 2015-01-28 Production Wells No Violations I inspected this well on 1/28/2015. The well was turned into production on 1/27/2015. Chesapeake Explorations company , Mark Tribadeux, indicated the initial flowing of the well was without incident. The flowing pressure of the casing was 3505 psi. with the tubing flowing at 2835 psi.. The well was flowing through a 16/64th choke manifold with volumes estimated at 3 mmcfg/d and 10 - 15 barrels of oil and 10 - 15 barrels of frac load. The 13.375 annular pressure gauge read 0 psi. The 9.625 annular pressure gauge had a reading of 200 psi. This anomaly will be addressed by relieving the annular pressure and see if there is a reoccurrence. The proper signage was placed at the wellhead. Tank battery, heater treaters and compressor staions spacing was in compliance.
