0766721613 2007-05-08 Plug / Plug Back No Violations Plug well. Well took significant amounts of water, 14,000 gallons, during drilling and clean out. Significant amounts of wood debris were washed out. Mix mud to circulate and reduce fluid loss. At 460 feet drilling on steel, drill 8 inches into object. Believed to be lost cable tool drill bit. Authorized plugging well with Class A Cement through drill pipe and bit. When trip out of hole pull 23 joints of 21 foot pipe and lost 17 joint, sub and bit in hole. Threads were stripped out. Drill pipe cemented in, well topped off with cement.
