| The Department was at the above referenced Marcellus Well to conduct a routine inspection.  This Marcellus Well has been drilled and fracked but is not in production.  The Department received a call from the Operator reporting a minor flow-back spill.  The Department observed a disturbed area where the flow-back water had been spilled and cleaned-up.  The Operator excavated the contaminated soil and should dispose of it at the proper disposal facility.  The disposed area has beeb re-graded and seeded and mulched.  Minuteman Spill Response was on site cleaning up the site during this inspection.  Minuteman was contracted by the Operator to conduct general clean-up activities such as disposing the synthetic liner that was placed on the site and any other waste on site other than drill cuttings and water.  The site is in over-all good condition.

| The Department was at the above referenced wells to conduct a CEI Inspection.  The Department issued the Operator a NOV on 8/4/11 after being notified of a release.  The Operator noticed stained rock on the well pad.  The Operator hired a Consultant to remediate the spill and conduct the cleanup.  The Consultant excavated 3 areas where they found contaminated soil.  The soil was screened with a PID as the excavation was being conducted.  Pre- excavation samples were taken to get an idea of what to sample.  Confirmatory soil samples were taken after the excavation to confirm cleanup.  The soil sample results were compared to Act 2 standards and appear to be in compliance.  The well pad had a liner placed on it so the Operator could complete some work on the well and utilize the liner as secondary containment.

| The Department was at the above referenced Marcellus Well to conduct a routine inspection.  This Marcellus Well has been drilled and fracked but is not in production.  The Department received a call from the Operator reporting a minor flow-back spill.  The Department observed a disturbed area where the flow-back water had been spilled and cleaned-up.  The Operator excavated the contaminated soil and should dispose of it at the proper disposal facility.  The disposed area has beeb re-graded and seeded and mulched.  Minuteman Spill Response was on site cleaning up the site during this inspection.  Minuteman was contracted by the Operator to conduct general clean-up activities such as disposing the synthetic liner that was placed on the site and any other waste on site other than drill cuttings and water.  The site is in over-all good condition.

| The Department was at the above referenced Marcellus Wells to conduct an inspection after being notified of a spill on the site.  The Department observed a wet spot on the rock on the well pad.  The spilled substance appeared to be an oily/organic material.  An unknown amount was spilled and impacted an area approximately 30 ft., by 10 ft.  The Operator had representatives on site to discuss the spill and the cleanup procedure.  The two Marcellus wells on site are in production.  The Department recommended finding the source of the spilled substance and to begin cleanup.  The Operator will need to excavate all impacted material and take confirmatory soil samples at the base of the excavation.

| The Department was at the above referenced wells to conduct a CEI Inspection.  The Department issued the Operator a NOV on 8/4/11 after being notified of a release.  The Operator noticed stained rock on the well pad.  The Operator hired a Consultant to remediate the spill and conduct the cleanup.  The Consultant excavated 3 areas where they found contaminated soil.  The soil was screened with a PID as the excavation was being conducted.  Pre- excavation samples were taken to get an idea of what to sample.  Confirmatory soil samples were taken after the excavation to confirm cleanup.  The soil sample results were compared to Act 2 standards and appear to be in compliance.  The well pad had a liner placed on it so the Operator could complete some work on the well and utilize the liner as secondary containment.

1992992 Primary Facility 2011-06-30 Routine/Complete Inspection Violation(s) Noted The Department was at the above referenced Marcellus Wells to conduct an inspection after being notified of a spill on the site. The Department observed a wet spot on the rock on the well pad. The spilled substance appeared to be an oily/organic material. An unknown amount was spilled and impacted an area approximately 30 ft., by 10 ft. The Operator had representatives on site to discuss the spill and the cleanup procedure. The two Marcellus wells on site are in production. The Department recommended finding the source of the spilled substance and to begin cleanup. The Operator will need to excavate all impacted material and take confirmatory soil samples at the base of the excavation.
2014123 Primary Facility 2011-10-20 Compliance Evaluation No Violations Noted The Department was at the above referenced wells to conduct a CEI Inspection. The Department issued the Operator a NOV on 8/4/11 after being notified of a release. The Operator noticed stained rock on the well pad. The Operator hired a Consultant to remediate the spill and conduct the cleanup. The Consultant excavated 3 areas where they found contaminated soil. The soil was screened with a PID as the excavation was being conducted. Pre- excavation samples were taken to get an idea of what to sample. Confirmatory soil samples were taken after the excavation to confirm cleanup. The soil sample results were compared to Act 2 standards and appear to be in compliance. The well pad had a liner placed on it so the Operator could complete some work on the well and utilize the liner as secondary containment.
