2483513 Primary Facility 2016-05-04 Incident- Response to Accident or Event In Compliance with Policy An estimated one to two barrels of production water was released from a production tank and directly into containment. The release was the result of a faulty weld and was discovered between stages of fracturing operations. The release was completely contained with no impact to water or soil, the Department was notified according to policy, the response was immediate and the remediation complete. The incident has been determined to be In Compliance with Policy (ICP). No violations are being cited at this time.
2487253 Primary Facility 2016-05-16 Administrative/File Review In Compliance with Policy The separation of a hose from a truck off-loading to an AST through a manifold system resulted in a release of brine at the Good Well Pad on Friday, May 13, 2016. The hose separation resulted from a mechanical failure and released an estimated ½ barrel of brine into containment. Spray from the hose, estimated to be less than a ½ barrel (1/4 to 1/2 barrel) fell beyond containment and impacted gravel on the well pad. The impacted gravel was excavated and disposed off-site. A conductivity probe was used to confirm that all impacted material was removed. The containment was vacuumed out and replaced with new materials. The impacted containment was disposed off-site. Plans are in place to take readings and sample underneath the AST when it is removed to verify that the impact did not extend to or beneath the tank.
