| On 4/20/2017, the Department performed an inspection of the Rice Energy Great Tiger well pad site. Present at time of inspection was Clay Clifford (Rice). The site is located in Aleppo Twp., Greene Co.  The inspection was a routine inspection post setting of conductors for newly permitted wells on the site.
 
 
 
The site is stable with greater than 70% vegetative cover, accept for a small slope failure in the PCSM structure on the northeast corner of the well pad. The operator will need to re-stabilize and repair the Retention Basin.
 
 
 
The operator has set 4 well conductors on the site in preparation for future drilling on these wells. Conductors have been set to 40' on the 2H, 4H, 6H, and 8H.

| On May 3, 2018, the Department performed an inspection of the EQT, Great Tiger well site. Present at time of inspection was Walter Nelson, and Jamie Davis (EQT). The site is located in Aleppo Twp., Greene Co. The inspection was related to a request by the operator for a minor modification to the Glass Joe,  ESCGP2 permit.
 
 
 
The operator has requested to enlarge the excess material stockpile along the Glass Joe and Great Tiger stockpile to allow for storage of the excess material/stone generated by the placement of the well production lines on the Great Tiger well pad. The minor modification was reviewed by the Department in the field and approved. The minor modification was redlined on the E&S plans for the ESCGP2 permit on the site.
 
 
 
A slide has developed in the well pad fill slope up gradient of the sediment trap on the northeast corner of the site. The operator is monitoring the soil movement and is currently developing a plan to stabilize the area.

2589088 Primary Facility 2017-04-20 Routine/Complete Inspection No Violations Noted On 4/20/2017, the Department performed an inspection of the Rice Energy Great Tiger well pad site. Present at time of inspection was Clay Clifford (Rice). The site is located in Aleppo Twp., Greene Co. The inspection was a routine inspection post setting of conductors for newly permitted wells on the site.

The site is stable with greater than 70% vegetative cover, accept for a small slope failure in the PCSM structure on the northeast corner of the well pad. The operator will need to re-stabilize and repair the Retention Basin.

The operator has set 4 well conductors on the site in preparation for future drilling on these wells. Conductors have been set to 40' on the 2H, 4H, 6H, and 8H.

2733932 Primary Facility 2018-05-03 Routine/Complete Inspection No Violations Noted On May 3, 2018, the Department performed an inspection of the EQT, Great Tiger well site. Present at time of inspection was Walter Nelson, and Jamie Davis (EQT). The site is located in Aleppo Twp., Greene Co. The inspection was related to a request by the operator for a minor modification to the Glass Joe, ESCGP2 permit.

The operator has requested to enlarge the excess material stockpile along the Glass Joe and Great Tiger stockpile to allow for storage of the excess material/stone generated by the placement of the well production lines on the Great Tiger well pad. The minor modification was reviewed by the Department in the field and approved. The minor modification was redlined on the E&S plans for the ESCGP2 permit on the site.

A slide has developed in the well pad fill slope up gradient of the sediment trap on the northeast corner of the site. The operator is monitoring the soil movement and is currently developing a plan to stabilize the area.
