| On 1/22/2019 at approximately 1100, I conducted an incident response inspection of the Cosgray well site located in Franklin Township, Greene County in response to a reported spill of produced water.

On 1/21/2019 at 1400, a valve leak as a mist was noticed by the operator. The operator approximated that less than 40 gallons of produced water released from a GPU as a result. Specialized Professional Services (SPSI) responded to remediate the impacted area.

| On 1/22/2019 at approximately 1100, I conducted an incident response inspection of the Cosgray well site located in Franklin Township, Greene County in response to a reported spill of produced water.

On 1/21/2019 at 1400, a valve leak as a mist was noticed by the operator. The operator approximated that less than 40 gallons of produced water released from a GPU as a result. Specialized Professional Services (SPSI) responded to remediate the impacted area.

During the 1/22/2019 inspection, the following was observed:

* Act 9 sign located at the site entrance was in good condition.

* Some secondary containment mechanisms utilized on 4 GPUs but containment appeared unable to hold 110% of the unit storage capacity.

* Liquid observed in the grated tray between the GPUs.

2833403 Primary Facility 2019-01-22 Routine/Complete Inspection Violation(s) Noted On 1/22/2019 at approximately 1100, I conducted an incident response inspection of the Cosgray well site located in Franklin Township, Greene County in response to a reported spill of produced water.

On 1/21/2019 at 1400, a valve leak as a mist was noticed by the operator. The operator approximated that less than 40 gallons of produced water released from a GPU as a result. Specialized Professional Services (SPSI) responded to remediate the impacted area.

During the 1/22/2019 inspection, the following was observed:

  • Act 9 sign located at the site entrance was in good condition.
  • Some secondary containment mechanisms utilized on 4 GPUs but containment appeared unable to hold 110% of the unit storage capacity.
  • SPSI was on site during the inspection cleaning the GPUs.
  • Liquid observed in the grated tray between the GPUs.
  • Well identification tags were not able to be found on any wells. An oil sheen was observed in one well cellar (37-125-27404).
