The inspection was related to a reported release, by the operator, of 3-5 barrels of crude/water mixture to the ground on 8/16/17.
The release was reported to the Harrisburg office of the Department within 2 hours of the release. While pumping a cement plug into the wellbore, the displacement water from the annulus was flowing into a wellhead cellar (sump) for capture and pumped out to a vacuum truck. During the operation, the vacuum pump temporarily failed and approximately four to five barrels of a crude oil/water mixture was displaced from the cellar and released onto the well pad (estimated 0.5 barrels of oil released with brine).
