| On April 24, 2017, the Department performed an inspection of the Noble Energy WFN3 well pad site. The site is located in West Finley Twp., Washington Co.  The inspection was a routine inspection during start of construction of the recently approved modifications to the ESCGP1 permit.
 
 
 
The operator has gotten a modification to the original ESCGP1 permit to leave the size of the well pad as constructed post restoration and to instal a newly engineered PCSMs (Retention Basin). Burns Excavating is on site to start construction of the Retention Basin. The operator has completed construction of the PCSMs (Infiltration Berms) along the access road. The site is stable with greater than 70% vegetative cover.

| On September 15, 2107, the Department performed an inspection of the HG Energy WFN3 well pad site. Present at time of inspection was John Carson (DEP). The site is located in West Finley Twp., Washington Co.  The inspection was a routine inspection of the site after restoration and prior to the NOT submittal by the operator.
 
 
 
HG Energy recently purchased the site from Noble Energy, but the transfer of the permits has not been processed by the Department. HG Energy is operating the site and wells as a third party consultant in Noble's name until the transfers are complete. Before selling the site to HG Energy Noble has modified the PCSM structures in the ESCGP2 permit. Installation of the PCSM structures and restoration of the site has been completed. The site is in good condition and stable. The access road and well pad are in good condition. The operator will need to remove the E&S controls on the site before submitting an NOT for the ESCGP2 permit for review by the Department.

| On April 24, 2017, the Department performed an inspection of the Noble Energy WFN3 well pad site. The site is located in West Finley Twp., Washington Co.  The inspection was a routine inspection during start of construction of the recently approved modifications to the ESCGP1 permit.

HG Energy is operating the site and wells as a third party consultant in Noble's name until the transfers are complete. 2589274 Primary Facility 2017-04-24 Routine/Complete Inspection No Violations Noted On April 24, 2017, the Department performed an inspection of the Noble Energy WFN3 well pad site. The site is located in West Finley Twp., Washington Co. The inspection was a routine inspection during start of construction of the recently approved modifications to the ESCGP1 permit.

The operator has gotten a modification to the original ESCGP1 permit to leave the size of the well pad as constructed post restoration and to instal a newly engineered PCSMs (Retention Basin). Burns Excavating is on site to start construction of the Retention Basin. The operator has completed construction of the PCSMs (Infiltration Berms) along the access road. The site is stable with greater than 70% vegetative cover.

The well pad and access road are in good condition.

4 wells have been drilled to TD and are equipped for production on site.

2640423 Primary Facility 2017-09-15 Routine/Complete Inspection No Violations Noted On September 15, 2107, the Department performed an inspection of the HG Energy WFN3 well pad site. Present at time of inspection was John Carson (DEP). The site is located in West Finley Twp., Washington Co. The inspection was a routine inspection of the site after restoration and prior to the NOT submittal by the operator.

HG Energy recently purchased the site from Noble Energy, but the transfer of the permits has not been processed by the Department. HG Energy is operating the site and wells as a third party consultant in Noble's name until the transfers are complete. Before selling the site to HG Energy Noble has modified the PCSM structures in the ESCGP2 permit. Installation of the PCSM structures and restoration of the site has been completed. The site is in good condition and stable. The access road and well pad are in good condition. The operator will need to remove the E&S controls on the site before submitting an NOT for the ESCGP2 permit for review by the Department.

2781042 Primary Facility 2018-09-27 Routine/Complete Inspection No Violations Noted On September 27, 2018, the Department performed an inspection of the HG Energy, WFN3 well pad site. Present at the time of the inspection was Tessa Antolick (ARM) and Jay Kendra (HG). The site is located in West Finley Twp., Washington Co. The inspection was related to a request by the operator to meet on site and review the site before submitting an NOT for the ESCGP2 permit.

The operator has installed the PCSM structures on the site and plans to terminate the ESCGP2 permit.

The well pad and access road are in good condition. The well pad has water pooling on the northeast side of the well pad, and should attempt to alleviate the pooling.

The Brine/Condensate Tanks and GPUs are in good condition with secondary containment.

The Infiltration Berm (PCSM) along the access road has been constructed and is functional. The operator will need to remove the silt fence (E&S BMP) prior to submitting the NOT for the ESCGP2 permit. The Retention Pond (PCSM) for the well pad has been constructed and is functiona
