2697630 Primary Facility 2018-02-23 Routine/Complete Inspection No Violations Noted Inspected this well for Application for Inactive Well Status. There appear to be 3 wells on this pad covered with steel plates and surrounded by steel fencing and concrete barriers. There is nothing above ground. As per Well Record drilling was completed on 9/23/13. The 30" conductor was set at 128 ft., 16" surface casing was set at 337 ft., 11 3/4" coal protective casing was set at 498 ft. and the 8 5/8" intermediate casing was set at 2224 ft. The well was drilled to 2245 ft.
2819315 Primary Facility 2018-12-26 Site Restoration No Violations Noted On 12/26/18 a restoration inspection of the Brunazzi-Sofran Site was conducted in response to an operator request for permit termination. The following was observed:

>>The site was well vegetated with no visible erosion or other violations.

>>No active unconventional wells or servicing equipment were present on site at the time of inspection, a nearby conventional well appeared to be functional and well maintained.

>>The accessory pad had been fully restored to pre construction conditions, the access road to the adjacent conventional well was intact.

>>Various agricultural equipment, materials, and a semi with trailer appeared to be in storage on the pad.
