The 18 5/8” casing was set at 657 ft. on 4/12/17.  The operator attempted to displace cement to surface however the top of cement was 37 ft. short of surface.  The operator then grouted the 18 5/8” casing to surface using 13 bbls with 8 bbls returned to surface.  The 13 3/8” casing was set at 931 ft. on 4/15/17 and grouted to surface above a basket.  A mine was found from 885 ft. to 892 ft.  A basket was set at 848 ft.  The 9 5/8” casing was set at 2583 ft. on 4/18/17 and cemented to surface with 52 bbls returned.  A Casing Annulus Packer was set in the 9 5/8” casing string from 1789 ft. to 1799 ft.  TD was at 5922 ft. on 4/20/17.
 
 
 
Note:  The Operator failed to circulate cement to surface despite running at least 120% volume of cement.  The Operator shall follow 78.83b requirements for lost circulation by choosing one of the 5 options listed below.

Proper signs were displayed identifying the site. EQT currently has a rig on site and is drilling 1 of 6 new wells. The well heads onsite are contained inside subsurface vaults covered with equipment for the drilling process and could not be visually inspected. The production tanks were in good condition no leaks , rust or defects observed. Secondary containment for the site was intact and maintained.
 
 
 
The site had some stabilization issues on the SW side of the access road.
 
I there was some slides forming inside and outside the LOD.  I spoke with Brian Morris from EQT about the issue and we arranged a site inspection on 3/13/18 at 1:30pm to discuss the plan for stabilizing that area.

| Pioneer Drilling Rig #72 moved onto this well on 3/18/18 and resumed drilling at 5922 ft. MD KB.  KOP was at 6030 ft.  EOB was at 8154 ft. MD (7585 ft. TVD).  TD was at 13,588 ft. MD (7643 ft. TVD) on 3/23/18.  The 5 1/2" casing was set at 13,572 ft.  Estimated TOC at 3926 ft.
 
 
 
Note:  There was a leak of oil-based mud from a storage tank on 3/23/18 at approximately 8:00 pm.  Approximately 10 bbls of fluid was lost from the tank into containment.  There were a couple small holes in the containment and 10 to 20 gallons of fluid went off containment.  A clean-up crew did the remedial clean up on 3/24/18.

The 18 5/8” casing was set at 657 ft. on 4/12/17.  The operator attempted to displace cement to surface however the top of cement was 37 ft. short of surface.  The operator then grouted the 18 5/8” casing to surface using 13 bbls with 8 bbls returned to surface.  The 13 3/8” casing was set at 931 ft. on 4/15/17 and grouted to surface above a basket.  A mine was found from 885 ft. to 892 ft.  A basket was set at 848 ft.  The 9 5/8” casing was set at 2583 ft. on 4/18/17 and cemented to surface with 52 bbls returned.  A Casing Annulus Packer was set in the 9 5/8” casing string from 1789 ft. to 1799 ft.  TD was at 5922 ft. on 4/20/17.

Note:  There was a leak of oil-based mud from a storage tank on 3/23/18 at approximately 8:00 pm.  Approximately 10 bbls of fluid was lost from the tank into containment.  There were a couple small holes in the containment and 10 to 20 gallons of fluid went off containment.  A clean-up crew did the remedial clean up on 3/24/18.
