Well initially owned (drilled) by T & F Exploration LP, transferred to Quest Oil and Gas, and now owned by Samson Resources since11/2008  Inactive status approved 5/22/09.  Site was properly signed. Access road was in good condition. Brine tanks appeared sound. No accelerated E&S or IW issues were observed. Site was restored with vegetative cover surrounding well and well pad rocked.  (Accompanied with, WQS Supervisor, Ed Ritzer at time of inspection).

| During inspection, DEP met Samson representative, Marvin Bartlett.  Well plug in accordance with 78.92 regulations.  Pressure initial on well was 1250 psi.  Well was removed of pressure and fluids sent to above ground transfer tank.  Marcellus well plugging.  Removed all tubing and jack pump from well.  Ran wire line on the well and confirmed cementing of well.  Top of cement on the 5 ½¿ was 1812 ft.  Surface casing (8 5/8¿) was set at 482 ft which was cemented to surface.  Samson set mechanical plug at 3440ft.  .  Process of setting cement plug of 100 ft on top of plug. (13 sks of class A). Top of cement plug was 3300 ft.  No pit was completed.  Samson using containers for water from well and potable water tanker truck.  At the time of inspection, no E&S issues and no IW discharge issues

| At time of inspection well is plugged. Plugging certificate received by Dept on 5-16-2011. Restoration report received on 10-3-2011 but, according to Dept records, not approved .Permit number affixed on metal plate on erected monument. Temp BMP's (silt fence) observed downgradient of plugged well. Site is permanently stabilized with 70% growth therefore Dept recommends Opr remove temporary BMP's from location. AR culvert does not appear to be maintained. Culvert pipe under AR appears to be filled with sediment therefore AR downgradient of culvert has eroded exposing culvert pipe. Downgradient of exposed culvert pipe, AR developing deep ruts.Inspection is PENDING. Viols may be forthcoming pending Dept investigation.UPDATE: Dept contacted Opr by phone on 12-28-15. Opr stated that the well was plugged and abandoned. Gated AR with well label attached and BMP's still on site therefore site not permanently restored as required after plugging a well.Violation being issued for failure to restore well site after plugging.

| FUI to Inspection Record #2435612 . AR gated. Site ID sign located near gate. Approx 300‘ from AR gate Wolf Camp Run crosses under AR through two, 36” culvert pipes. It appears a small amount of water from Wolf Camp Run is not going through the culvert pipes but is crossing over AR resulting in erosion. It appears the streambed of Wolf Camp Run has overflowed and not all water is being directed into culvert pipes. Continuing down AR approx 450' from the two 36" culvert pipes, the culvert pipe mentioned in Inspection Record #2435612 remains filled with sediment. The AR remains washed out exposing approximately 13' of culvert pipe that should be covered and under the AR. This culvert pipe appears to be 14” in diameter.The deep ruts and erosion along and on the AR down gradient of this culvert pipe appears to be due to the failure of this culvert pipe are still present. Temp BMP’s (silt fence and stakes) remain on site. E&S plan not found on site.An outstanding violation still exists for this site.

Landowner stated that, to his knowledge, there was never a written agreement.

1963841 Primary Facility 2011-04-06 Plugging(Includes Plugged/Mined Through) No Violations Noted During inspection, DEP met Samson representative, Marvin Bartlett. Well plug in accordance with 78.92 regulations. Pressure initial on well was 1250 psi. Well was removed of pressure and fluids sent to above ground transfer tank. Marcellus well plugging. Removed all tubing and jack pump from well. Ran wire line on the well and confirmed cementing of well. Top of cement on the 5 ½¿ was 1812 ft. Surface casing (8 5/8¿) was set at 482 ft which was cemented to surface. Samson set mechanical plug at 3440ft. . Process of setting cement plug of 100 ft on top of plug. (13 sks of class A). Top of cement plug was 3300 ft. No pit was completed. Samson using containers for water from well and potable water tanker truck. At the time of inspection, no E&S issues and no IW discharge issues
2435612 Primary Facility 2015-12-09 Routine/Complete Inspection Violation(s) Noted At time of inspection well is plugged. Plugging certificate received by Dept on 5-16-2011. Restoration report received on 10-3-2011 but, according to Dept records, not approved .Permit number affixed on metal plate on erected monument. Temp BMP's (silt fence) observed downgradient of plugged well. Site is permanently stabilized with 70% growth therefore Dept recommends Opr remove temporary BMP's from location. AR culvert does not appear to be maintained. Culvert pipe under AR appears to be filled with sediment therefore AR downgradient of culvert has eroded exposing culvert pipe. Downgradient of exposed culvert pipe, AR developing deep ruts.Inspection is PENDING. Viols may be forthcoming pending Dept investigation.UPDATE: Dept contacted Opr by phone on 12-28-15. Opr stated that the well was plugged and abandoned. Gated AR with well label attached and BMP's still on site therefore site not permanently restored as required after plugging a well.Violation being issued for failure to restore well site after plugging.

2462913 Primary Facility 2016-03-09 Follow-up Inspection Outstanding Violations - No Viols Req'd FUI to Inspection Record #2435612 . AR gated. Site ID sign located near gate. Approx 300‘ from AR gate Wolf Camp Run crosses under AR through two, 36” culvert pipes. It appears a small amount of water from Wolf Camp Run is not going through the culvert pipes but is crossing over AR resulting in erosion. It appears the streambed of Wolf Camp Run has overflowed and not all water is being directed into culvert pipes. Continuing down AR approx 450' from the two 36" culvert pipes, the culvert pipe mentioned in Inspection Record #2435612 remains filled with sediment. The AR remains washed out exposing approximately 13' of culvert pipe that should be covered and under the AR. This culvert pipe appears to be 14” in diameter.The deep ruts and erosion along and on the AR down gradient of this culvert pipe appears to be due to the failure of this culvert pipe are still present. Temp BMP’s (silt fence and stakes) remain on site. E&S plan not found on site.An outstanding violation still exists for this site.
2469695 Primary Facility 2016-03-22 Routine/Partial Inspection Outstanding Violations - No Viols Req'd Routine partial inspection as a follow up to Inspection Record #2435612 where violation issued for failure to restore. Inspection conducted to verify that water crossing AR at the two 36" culvert pipes is overflow water from Wolf Camp Run.

At time of inspection site ID sign present. Access road gated. Approximately 300' from access road gate a small amount of water from Wolf Camp Run is leaving stream bed approximately 100 feet west of access road. This water crosses access road approximately 30’ north of two 36” culvert pipes instead of flowing through the two culvert pipes. Erosion gullies beginning to form on access road north of 36" culvert pipes. Department recommends Operator continue to monitor and maintain access road to avoid any E&S issues. A previously issued violation still exists for this site.

2480730 Primary Facility 2016-04-21 Follow-up Inspection Outstanding Violations - No Viols Req'd Follow up inspection to inspection Record ID #2435612 where violation issued for failure to restore well site within 9 months of plugging a well.

At time of inspection well tag present on erected monument. Gate at beginning of access road and ID sign near gate remain on site. Silt fence downgradient of well site remains on site. Access road remains eroded and culvert pipe remains unmaintained, as noted in previous inspections. A previously issued violation still exists for this site.

2508808 Primary Facility 2016-07-26 Administrative/File Review Outstanding Violations - No Viols Req'd Admin inspection to inspection record #2435612 where violation issued for failure to restore well site after plugging.

According to letter mailed to DEP Assistant Counsel Rick Watling dated 5-26-16, Operator feels an agreement was made with landowner to allow the access road to remain after plugging. To verify this, the Department spoke to the landowner by phone to determine if a written agreement was made between landowner and Samson Resources. Landowner stated that, to his knowledge, there was never a written agreement. Department requests that the Operator provide documentation of alleged agreement with landowner or remove/restore access road according to current regulations.

2526723 Primary Facility 2016-09-28 Administrative/File Review Outstanding Violations - No Viols Req'd Admin inspection to inspection record #2435612 where violation issued for failure to restore well site after plugging.

Department received communication from landowner via e-mail. Landowner states that Operator agreed to restore access road to its original state/pre-existing uses after plugging well and abandoning site but did not provide written proof of such agreement. Therefore, since well has been plugged, Operator shall restore access road to approximate original contours in accordance with regulations.

2536146 Primary Facility 2016-11-03 Routine/Partial Inspection Outstanding Violations - No Viols Req'd Routine Partial Inspection to Inspection Record #2435612 where violation issued for failure to restore.

At time of inspection, erosion gullies remain on access road. It does not appear that Operator has taken any action to remediate access road E&S issues or to restore access road to near original contours. Previously issued violation still exists for this site.

2751936 Primary Facility 2018-07-13 Follow-up Inspection Outstanding Violations - Viols Req'd FUI to inspection record #2435612 where a violation was issued for failure to restore well site.

At time of inspection site ID sign and gate at access road entrance remain on site (photo #1). Two 36" culvert pipes at Wolf Camp Run have failed (photo #2) and water is flowing around these two pipes and across the access road (photo #3). Erosion gullies observed in inspection record #2462013 have worsened (photo #4). The culvert pipe approximately 450' from the two, 36" culvert pipes remains clogged. This clogged culvert pipe is now more exposed and contributing to accelerated erosion (photo #5). Temporary BMP'S remain on site (photo #6). Violations issued on 12-9-15 remains outstanding. A follow up inspection will be conducted at a later date.
