A tear was observed in the liner of containment being used for the storage of a number of barrels of fluids and pallets of dry material. The liner should be patched to ensure the proper functioning of the containment in the event a spill or release of the stored materials occurs. The Department requests that Chief provide photographic docuemtnation that this tear has been repaired. No violations were noted during this inspection. |

During the inspection, several housekeeping items were identified, as described in this report. I informed Chief HSE representative Joshua Gresch of my findings. He immediately had a suction truck begin vacuuming the puddles on the pad surface on which sheen was observed. The flat-bed truck with sheen on the bed was being cleaned with adsorbent materials. The barrels stored on the truck may be empty, but if not will be moved to an appropriate storage location within containment. No violations were cited during this inspection. |

There were 3 properly tagged Marcellus wells staged for production. The well cellars were filled with gravel. The Baumunk 1H annulus was plumbed to an ~350 gallon poly tank and the Czop 1H and 2H were plumbed to a 550 gallon poly tank. The tanks were not on additional secondary containment. There were 3 GPUs, 3 produced water tanks, and 1 fresh water injection tank onsite and in secondary containment. It did not appear that the fresh water tank was labeled. Two detention basins were recently installed/modified. These areas were not yet vegetated but they were temporarily stabilized with straw and erosion control matting. The matting was rolled in several areas and may need to be repositioned (Photos #3-6). These areas also had CFS and straw bales as E&S controls. Other areas of the pad were well vegetated and no E&S issues were noted. The Department recommends repositioning and securing |
