**Note: This Inspection Record replaces Administrative Inspection Record 2536466, which has been rescinded.  After further investigation, the Department has determined that there are two wells named Gremmel 5H on the Gremmel Well Pad.  The original Gremmel 5H was reportedly spud under well permit 115-20774, but was subsequently reported as ORND by SWNPC.  SWNPC later submitted a renewal permit application for the Gremmel 5H, but because the Gremmel 5H was listed as ORND a new well permit number was assigned (115-21260).  Permit 115-21260 superseded permit 115-20774, even though the wells had two different SHLs.  The spud date associated with 115-20774 was then inadvertently carried over to 11

| On February 1st 2017 at 12 pm I conducted a routine inspection of this location. There are 2 wells on the pad; the 6H (in production) and the 5H (conductor). I received a Notice of Intent to plug the 5H. Operations were scheduled for this week. I received a call from the operator on the way up who said operations will be pushed back to next week when they have a contractor available. I discussed their proposed procedure; no issues. At the time of inspection; the location was snow covered. The 5H was capped. The cellar was open and there was no visual or audible bubbling in the cellar. The 6H had a production tree in place. There was 454 PSI on the production casing and 378 PSI on the tubing. The cellar was filled with gravel. No obvious concerns noted at the wellhead. No violations noted during this inspection

| On February 7th 2017 at 11 am I conducted an inspection of the Gremmel 5H (115-20774) now plugged 87’ conductor. At the time of inspection; the 5H 20” conductor was cut down below ground level in the cellar, capped and labeled. The cellar was full of muddy water below conductor cap; no bubbling. Cement was poured in the 87’ conductor casing and capped yesterday. There were no obvious concerns.
