2564548 Primary Facility 2017-02-22 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 14:50p.m.

The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water above the surface casing with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure and the valve going to the production tank was closed. There was 443psi on the production casing and 345psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 15:20p.m.

2660581 Primary Facility 2017-11-09 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a corrugated pipe around the well for inspection access. There was water above the (20x13) and (13x9) annuli with no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2732670 Primary Facility 2018-05-21 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. No E&S issues were observed. No violations noted.
2759373 Primary Facility 2018-08-02 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water 2' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
