2268118 Primary Facility 2014-04-29 Preoperation Inspection No Violations Noted The Department was on site at the SWN NR11 Well Site to conduct a Preoperational Inspection. Inspection was conducted with Rich Gulyas and Andy Breese. The well permit issued for this pad is the Dayton Pad (115-21607). All perimeter BMPs are in place including red lined additions. Pad is 90% complete. Culvert C-7 inlet slightly compressed due to heavy truck traffic in area but is currently being repaired. No violations noted.
2284363 Primary Facility 2014-06-27 Routine/Complete Inspection No Violations Noted Precision 532, well shut in. Liner on location. Well not fract. No visible bubbling in cellar. No gauges on annuli

2480212 Primary Facility 2016-04-25 Routine/Complete Inspection No Violations Noted On 4/25/16, the Department conducted a routine inspection of the SWN NR 11 Dayton Pad. The Department observed good temporary stabilization. Vegetation is well established. Only one of the 3 wells that have been drilled on this pad has been stimulated and placed into production, the Dayton 4H, 115-21607. The 5H, 115-22069 and the 6H, 115-22070 have been drilled only. Temporary containment is still in place around the well heads. A cage has been placed around the 4H. The pipeline tie-in has good temporary stabilization. No violations noted.

2569507 Primary Facility 2017-03-06 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:00p.m.

There was containment around the well that made for limited access to the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge and a 1000psi pressure gauge with 0psi on it, the valve going to the production tank was closed. There was 561psi on the production casing and 322psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 12:30p.m.

2665131 Primary Facility 2017-11-29 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was containment around the well and was unable check the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2679185 Primary Facility 2018-01-05 Site Restoration No Violations Noted I inspected the Northwest Unit 11 well pad for Notice of Termination (NOT) inspection. The pad has 1 active well on site, the 4h, permit #115-21607. There are two wells, the 5h and 6h which are in Regulatory Inactive status. Their permit #s are 115-22069 and 115-22070, respectively. The site was overall in good shape with no erosional issues present at the time of inspection. The vegetation which was appearing through the snow cover appeared full. The wells(s) were properly labeled.

There were no violations observed.

I recommend APPROVAL of this NOT.

2766100 Primary Facility 2018-08-22 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was containment around the well but I was able to notice the cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water .5' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2793254 Primary Facility 2018-10-11 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. Entire site has good stabilization. No violations noted.
