| Falcon Rig# 18 is the active drilling rig on the location.  Laddie Pendergraft is the company man on location.  The current drilling depth is (4,740) feet.  I observed a relatively large hole in the composite mats surveying the drilling rig containment system.  Beneath the composite mats are layers of felt and a (40) mil HDPE liner.  Mr. Pendergraft said the hole will be repaired today.  The secondary containment as part of the drilling rig and associated equipment appeared to be in good condition as well as did the production equipment secondary containment.  The fluid levels in the drill cuttings containers and Keystone Landfill containers did not exceed the (2)’ freeboard requirement of Title 25, Env. Protection, Pa. code 78.56.

| Arrived at the site at 12:00 in response to reported spills of (3-5) barrels of flowback water and (50) gallons of antifreeze.  Hydraulic fracturing operations were in progress.  Keane Frac is the subcontractor conducting well stimulation (frac) procedures on Innes South 8H well.  Justin Phillips, Health, Safety and Environmental Staff Coordinator Supervisor of SWN reported the estimated (3-5) barrel capacity spill flowback water spill that occurred on containment to the Department at 9:30 PM on 12/13/14.  Chris Becker, Health, Safety & Environmental staff Coordinator of SWN reported the estimated (50) gallon antifreeze spill to the Department on 12/14/14 at 2:30 PM.  There is no evidence the spills escaped secondary containment, no evidence of comprised liner integrity and no evidence of an impact to the pad surface in the general area where the spills occurred.  Both spills occurred due to mechanical malfunction according to SWN.

| Administrative Inspection follow-up to RTNC Inspection 2561203.  The Innes North 4H was spud July 1, 2014, the well permit expired June 6, 2015 and no drilling activity has occurred since the well was spud.  Because the Innes North 4H has not been drilled to total depth to enable production of gas since the well was spud in June 2014, the Innes North 4H is therefore an abandoned well, which SWN has failed to plug.

The Innes 4H was plugged 5/17/2017.  The Plugging Certificate was received 6/30/2017 and was reviewed/approved on 7/3/2017.  VIOL resolved as CRCTD and Enforcement Record 351093 updated to Comply/Closed on the date the PLUGC was approved (7/3/2017).  SWNPC indicated in their NOV response that the Surface Hole Location (SHL) for the Innes 4H conductor was inadvertently reversed with the SHL for the Innes 8H (115-21799), so SWN will submit a permit modification to reflect the accurate as-drilled conditions for the Innes 8H.  ~CMB~

The 4H was conductor only. Original permit date was 6/6/2014. Expiration date was 6/6/2015. Spud date was 7/1/2014. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:10p.m.

2562295 Primary Facility 2017-02-16 Administrative/File Review Violation(s) Noted Administrative Inspection follow-up to RTNC Inspection 2561203. The Innes North 4H was spud July 1, 2014, the well permit expired June 6, 2015 and no drilling activity has occurred since the well was spud. Because the Innes North 4H has not been drilled to total depth to enable production of gas since the well was spud in June 2014, the Innes North 4H is therefore an abandoned well, which SWN has failed to plug.

The Innes 4H was plugged 5/17/2017. The Plugging Certificate was received 6/30/2017 and was reviewed/approved on 7/3/2017. VIOL resolved as CRCTD and Enforcement Record 351093 updated to Comply/Closed on the date the PLUGC was approved (7/3/2017). SWNPC indicated in their NOV response that the Surface Hole Location (SHL) for the Innes 4H conductor was inadvertently reversed with the SHL for the Innes 8H (115-21799), so SWN will submit a permit modification to reflect the accurate as-drilled conditions for the Innes 8H. ~CMB~
