2444421 Primary Facility 2016-01-26 Incident- Response to Accident or Event No Violations Noted Steve Watson, Environmental Health and Safety Specialist of Cabot Oil and Gas contacted PADEP Northeast Regional Office Emergency Response Team at 4:45 PM on 1/25/16 to report the discovery of staining on the pad surface on 1/25/16. NERO ERT contacted the Scranton District Office at 9:10 AM on 1/26/16 to report the discovery. Gene Rickard and I arrived at the site at 11:30 to conduct an inspection. There was no activity or personnel at the site as we arrived. Shortly upon arrival we observed an area of dark staining at N- 41.74130°; W- 75.69933°, on the pad surface near where Diaz roll-off containers were stored on containment. The substance had a petroleum odor to it. Andy Mehalko of Cabot indicated that Cabot had discovered the staining while in the process of removing the liner from the pad surface used during drilling operations. It is believed that the staining was a result of compromised liner integrity and consisted of a mixture of oil and water based drilling mud.

2507151 Primary Facility 2016-08-02 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:25a.m., Pumpco Services was on site and fracturing the wells. I took a walk around the tank farm and all the tanks were on containment and the containment was in good condition. They were pumping water through a 12" plastic line from the lower pad to the upper pad. I met with company men Rafael Rodriguez, Larry Fulmer and Jim Crouch, they were fracturing stage 2 on the 9H and running a plug and perforating the 1H.

They have completed: (5 of 32 on the 1H), (5 of 32 on the 3H), (1 of 27 on the 5H), (0 of 18 on the 7H), (1 of 28 on the 9H), and (8 of 35 on the 11H). I took a walk around the location and everything looked OK. I left the site at 10:30a.m.

2527864 Primary Facility 2016-09-26 Routine/Complete Inspection No Violations Noted As I arrived at the site at 11:40, Flowback of the wells was in progress. I initially met with Eric Bronson and Nick White of Rain for Rent on the tank pad of the site. Mr. Bronson and Mr. White stated that there are (28) frac tanks in the tank farm on the tank pad containing fluid. The tanks contain flowback and frac plug drill out water from the wells on this pad. The water was pumped from the well pad to tank pad through a (12) inch HDPE line. The frac tanks are manifolded. The containment for the tank farm on the tank pad appeared to be in good condition. Conductivity was in the range of (9-10) milliSiemens/cm³ in water in the duck pond containment for one of the Rain for Rent pumps in the tank farm.

I met with Donnie Griffith, site company man and consultant of Cabot on the well pad. Mr. Griffith stated the following. "Flowback started on the 9/16/16. All wells are in the process of being flowed back with natural gas sent directly to sales. Flowback of the wells will take place for another week

2530400 Primary Facility 2016-10-19 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:20p.m., the wells were recently completed and are now in production. The cellar had water at the bottom with no bubbling noticed, the (20x13) vent pipe was open and venting into a tank, the (13x9) vent pipe was open and venting into a tank, the (9x5) vent pipe was open and venting into a tank, there was 672psi on the production casing. The well tag was on the well and correct. The production fluid was going into a 500bbl flow back tank that was on containment and the containment was in good condition. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 13:10p.m.

2531667 Primary Facility 2016-10-25 Routine/Complete Inspection No Violations Noted On 10/25/16, the Department conducted an inspection of the COG Ferguson A Pad located in Harford Township, Susquehanna County. The 6 wells associated with this pad, the 1H, 3H, 5H, 7H, 9H, and 11H have been drilled, stimulated, and recently placed into production. Most of the drilling and stimulation temporary containment has been removed from the site. 4 large roll off containers filled with what appears to be virgin frac sand is still present on the pad. All permanent production equipment has been set in place and appears to be functioning. The large top soil stock pile located on the left side of the access road is stable, with moderate vegetative growth. No violations noted.

The cellar was about a quarter full of water with no bubbling noticed. The (20x13) annulus vent pipe was open and venting into a small plastic tank, the (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. All the annulus vent pipes were connected to one pipe going into the same tank. There was 514psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 9:50a.m.

2672717 Primary Facility 2017-12-11 Incident- Response to Accident or Event No Violations Noted I arrived at the site at 11:11 in response to a reported spill of an estimated (5-6) gallons of production fluid to the pad surface. I was accompanied by Matt Shope, DEP O&G WQS during the inspection. The weather conditions were mostly cloudy skies, breezy and cold with snow flurries and temperatures around 32 degrees. There was no activity or Cabot personnel at the site during the inspection. There are (7) wells in production, the odd numbered wells, 1H, 3H, 5H, 7H, 9H and 11H. Shortly upon arrival, we observed a drum staged next to the steps of the containment structure for the production fluid tanks. We had suspected that this drum contained impacted material associated with the production fluid spill and the area of impact was in a close proximity to the drum and containment structure. All gas checks were conducted with a MSA Altair 5X multi gas detector that was calibrated on 1/2/19. I took a walk around the location and there were no obvious concerns noticed.
